Cost Analysis And Relevant Problem Research About Drgs Of Common Cancers In The Provincial-level General Hospital Of Anhui Province

Objective: After the payment of common cancers included in the DRGs by the provincial-level general hospital in Anhui Province,discuss the impact on the fee structure and Pays ratio,and study the relevant problems encountered and the satisfaction degree of patients during the implementation of the hospital policy,which can provided a reference for hospital management department to develop and improve the relevant policies.Methods: Four types of cancer,esophagus,stomach,breast and lung cancer,are selected from the HIS system of a provincial hospital in Anhui province.Analysis and compare of the cost change before and after the implementation of DRGs,and discuss the trend,reason and existing problems of cost change.By questionnaire survey,live interviews and telephone interviews,investigate the relevant staff who perform the work in the hospital.Results: Compared with fee structure before the implementation,the index value of cost constitute of the four kinds of diseases have the overall downward trend;the part index value of fee structure rise;average cost per time change significantly.The proportion of self pay of patients decreases significantly.The proportion of breast cancer,lung cancer,esophageal cancer,stomach cancer decrease about 20%.The difference have statistical significance(P<0.05).The survey staff are more satisfied with the pay way reform,but there are some staff have objections Patients generally consider that the pay reform reduces their medical burden,and the treatment effect is also positive,but there are still some patients hold the different views.Conclusion: After the implementation of new policy in Anhui Province,Fee structure proportion have downward trend,and self pay ratio significantly reduced.The farmer patient's burden is effectively reduced,and the NCMS funds are reasonably and effectively used.However,there are still some problems in the implementation process,and this is the necessity of reform and development.With the reform and development gradually improved,After the implementation of the DRGs policy,the effect is worthy of recognition.
