| Objective:Vascular dementia is an important problem in clinical practice,and get increased attention.But it is still lack of objective and quantitative diagnostic indicators and effective treatment.In this study.Through the use of Magnetic Resonance Spectroscopy(MRS)examination method,try to explore the effectiveness of Modified Shuyu Pill in the treatment of mild to moderate vascular dementia and mechanism of action.Methods:This study was included in the Hubei Provincial Hospital of Traditional Chinese Medicine in December 2015 to December 2016 outpatient and inpatient compliance with the standard of a total of 80 subjects,including vascular dementia patients 60 people(the Kidney essence deficiency group(n=20),Phlegm turbid blocking upper orifices group(n=20)and Blood stasis obstructing brain collaterals group(n=20))and the normal control group of 20 people.80 subjects were given neuropsychiatric assessment,TCM syndrome assessment,liver and kidney function,electrocardiogram and other general examination and brain bilateral thalamus,frontal lobe MRS examination before treatment.60 Va D patients in the treatment of underlying diseases at the same time,to be 30 ml of Modified Shuyu Pill daily,treatment for 12 weeks.Observed:(1)The MRS difference between normal group and in patients with vascular dementia before treatment;(2).The changes of MMSE、SDSVD and brain MRS in patients with Va D after treatmentand and adverse reactions.Results: 1.During the follow-up of the subjects,Kidney essence deficiency group patients fall off 2 cases,Phlegm turbid blocking upper orifices group patients fall off 3 patients,Blood stasis obstructing brain collaterals group patients fall off 3 cases.2.Compared with the normal group,in the right side of the middle frontal gyrus,the Cho/Cr value of the three groups of patients with Va D was higher than that of the normal group(P<0.01),NAA/Cr values were decreased in different degrees compared with the normal group and were statistically different.In the left side of the middle frontal gyrus,can also be observed in the right side of the middle frontal gyrus.The Cho/Cr value of Va D patients was higher than that of normal group(P <0.001),NAA/Cr values were decreased in different degrees compared with the normal group(P<0.05).There were significant differences between the two groups(P<0.05).3.Compared with the three groups of Va D patients,in the right side of the middle frontal gyrus,The NAA/Cr value of patients with kidney essence deficiency group patients was significantly lower than that of patients with Blood stasis obstructing brain collaterals group(P = 0.034)and Phlegm turbid blocking upper orifices group(P = 0.006).4.The MMSE score and SDSVD score of Va D patients were significantly improved after treatment(P <0.05).5.After treatment of three groups of Va D patients,in the right side of the middle frontal gyrus,the NAA/Cr values were higher than before treatment were significantly(P <0.001,);In the left side of the middle frontal gyrus,NAA/Cr value only kidney essence deficiency group patients than before the rise(P= 0.026).but compared with the normal group was still statistically significant P> 0.05.6.Three groups of Va D patients with blood,urine,stool routine,liver function,renal function tests and ECG examination in the course of the test did not find abnormal circumstances.Conclusion: 1.1H-MRS can be used as a noninvasive and objective way of examination for the diagnosis and evaluation of patients with vascular dementia,And to reveal that vascular dementia may be related to abnormal metabolic function of frontal lobe neurons.2.Modified Shuyu Pill is safe and effective way to improve the cognitive function of patients with vascular dementia,but also from the whole,to improve the patient’s other symptoms.3.Modified Shuyu Pill to improve the cognitive function of patients with vascular dementia may be related to the repair of neuronal function. |
