Comparison Of The Effects Of Two Kinds Of Anal Fistula Operation On The Anal Function Which Based On Anorectal High-resolution Detection

Goal:The low position incisin and skin-retention and high position with thread-drawing therapy is the major treatment of high complex anal fistula curative effect,but it has the postoperative wound,and also has different effects on anal function.Our department through the traditional operation improvement,with the low position incisin and skin-retention and high position with thread-drawing technique in the treatment of high complex anal fistula,which has a good curative effect,and better protect the anal function,but it lack of objective data support.High resolution anorectal manometry,high resolution and accurate data,can be used to evaluate the fine function of anus.Through the detection of the two kinds of preoperative and postoperative anal function related indexes,the influence of the operation on the anal function was evaluated,and the superiority of the low position incisin and skin-retention and high position with thread-drawing technique was verified.Methods:40 cases of patients with high complex anal fistula were analyzed and divided into two groups in random,20 cases of treatment group and 20 cases of control group.The treatment group(20 cases)was treated by the operation of low incision open skin bridge high hanging line,The control group(20 cases)was treated with low incision and high hanging line.By observing two groups of anorectal dynamics in healing time,pain,anal function(leakage,leakage,leakage case)and recurrence rate,and the results were statistically analyzed.Results:(1)there was no significant difference between the treatment group and the control group after treatment(P>0.05)(2)there was no significant difference in pain scores between the two groups after take operation 1 days,and the pain of the treatment group was lower than that of the control group on 7,14 and 21 days.(3)there were significant differences between the two groups in the postoperative anal function(leakage,leakage,and leakage)after take operation 3 months.(4)two groups of rectoanal contract reflex and rectoanal inhibitory reflex after 3 months,half a year after operation showed no significant difference.(5)After operation 3 months,compared with preoperative,two groups of anal sphincter resting pressure%anal average systolic pressure,and maximal squeeze pressure were decreased,There was no statistical difference in half a year after the treatment,There was statistical difference between the 2 groups after the operation 3 months and half a year after operation in the two groups.(6)After operation 3 months,compared with preoperative,two groups of sensory threshold?rectal maximal capacity were decreased,There was no statistical difference in half a year after the treatment,There was statistical difference between the 2 groups after the operation 3 months and half a year after operation in the two groups.(7)After operation 3 months,compared with preoperative,two groups of Anal sphincter function length was decreased,There was no statistical difference in half a year after the treatment,There was statistical difference between the 2 groups after the operation 3 months and half a year after operation in the two groups.(8)The treatment group,on average wound healing time was(47.80±6.092)days,the control group on average healing time was(54.20±4.629)days,the two groups have significantly difference on the healing time(P<0.05).Conclusion:The low position incisin and skin-retention and high position with thread-drawing therapy as the traditional mainstream technique,high complex anal fistula curative effect,but have different effects on anal function.the low position incisin and skin-retention and high position with thread-drawing technique is innovation operation in our department in the traditional operation basis,in the protection of anal sphincter and anal function,reduce postoperative pain,promote postoperative wound the healing of superiority,the maximum extent to alleviate the suffering of patients,improve the quality of life of patients after operation.
