An Empirical Study On The Svaluation Of Clinical Pathway Management In A Hospital Of TCM In Guangdong

ObjectiveIn this paper,by analyzing and summarizing the clinical pathway management evaluation experience,the selection of a set of scientific and effective evaluation to the hospital of traditional Chinese medicine clinical pathway management index,the empirical research on a Chinese Medicine Hospital of Guangdong Province,evaluation of Hospital of traditional Chinese medicine clinical pathway implementation process and implementation effect,find out the existing problems and countermeasures of clinical path management mode of traditional Chinese medicine in the hospital,so as to strengthen the hospital medical staff to pay attention to the clinical pathway management,to further promote and enhance the management level of Hospital of traditional Chinese medicine clinical pathway.MethodsThis paper combined with the research results,through a large number of relevant references,it selects a set of scientific,authoritative,high applicability of clinical pathway management evaluation index system.Second,through expert interviews,select group of experts and project office is responsible for the efficacy of Guangdong Province Traditional Chinese Medical Hospital,interview on how to communicate the clinical pathway Hospital of traditional Chinese medicine scientific and effective evaluation of the implementation process and results,for the selection of appropriate clinical and road hospital operational management direction size evaluation index.Third,an empirical study of a hospital in Guangdong Province,the statistical data were collected and analyzed by comparative analysis,analyzed the overall effect of the implementation of clinical pathway management in the hospital in 2015 and 2016,contrasted the implementation of clinical pathway management between different clinical pathway in 2016,analysis of the optimal clinical pathway.ResultsDetermined 15 indicators for evaluating the implementation of clinical pathway management in TCM hospital,empirical study is carried out and the results show:the average rate of disease path of clinical pathway management was 60.39%in the empirical hospital in,which was significantly higher than the mean value(51.20%)of the year 2015,after an independent sample t-test,P<0.05,the difference was statistically significant;In each clinical pathway grouping,the highest mean rank of the disease path rate(positive index)in the clinical pathway of atrial fibrillation in cardiovascular department,the average drug cost(negative index)was the lowest in the clinical pathway of colorectal polyps in the digestive department,and in the rank sum test,P<0.05,the difference was statistically significant.ConclusionThe results of this study show,the overall evaluation of clinical pathway management in 2016 year was better than that in the year of 2015.The efficiency of the implementation of clinical pathway has been improved,in which the disease rate and path completion rate have been improved,but the path completion rate is not high,the path variability rate is higher and has increased,so we should strengthen the prophase management and the later summary of the clinical pathway implementation,and improve the clinical path management executive power.The effect of the clinical pathway implementation achieved good development,continue to reduce the average hospitalization days,mortality rate steadily control,clinical pathway effect more standard,but the readmission rate has a slight increase,should further standardize the treatment process and improve the quality of medical care.Strengthening the application of traditional Chinese Medicine,Chinese herbal pieces,proprietary Chinese medicine and traditional Chinese medicine therapy in clinical pathway management has been widely used,but the cost of Chinese medicine is relatively low.we should further highlight the advantages of traditional Chinese medicine and promote the social and economic benefits of traditional Chinese Medicine.Medical supplies are well charged,however,the average medical costs have been rising,the process of diagnosis and treatment should be further standardized,to strengthen the monitoring of hospital expenses and drug costs.The informatization construction of clinical pathway is not perfect.It is necessary to combine the clinical pathway with the electronic medical record effectively,and focus on the management of the system path.Moreover,in 2016,the incidence rate of the clinical pathway of atrial fibrillation was the highest,and the clinical pathway of colorectal polyps was the best.
