Purpose: To evaluate the safety and long-term outcomes of repairing benign biliary stricture of bile duct with autologous vascularized pedicled flap of stomach tissue. Methods: Retrospective analysis of using autologous vascular pedicled flap of stomach tissue to repair benign biliary stricture cases from 2002 to 2017. Duration of operation, hemorrhage, postoperative complications were summarized and analyzed to illustrate the safety of the operation. Long-term biliary tract symptoms, liver function and recurrence of biliary stricture were recorded. Results: 54 patients were enrolled in this study. 55 patients with benign stricture of the bile duct were repaired with autologous vascular pedicle flap (one patient underwent twice repaired operation). The average operation time was 253.1 ±60.7 min,the median intraoperative hemorrhage was 200.0(100.0,200.0)ml, the average postoperative hos-pital stay was 12.7 ± 7.2 days. The overall complication rate was 20.0%(11/55),delayed healing of incision in 6 cases, biliary fistula in 2 cases, ascites in 1 case, infection of biliary in 1 case, biliary bleeding in 1 case. The median follow-up time was 78(70,104)months and following rate was 100%. In this study the excellent rate was 94.1%(48/51),3 patients died of malignant tumor, 3 patients reoperation, 2 cases liver function was abnormal, which was improvement after the treatment of choleretic drug. The remaining 46 patients showed normal liver function and without of abdominal pain, biliary stricture. Conclusion: It is a safe and reliable method to repair benign biliary stricture using autologous vascularized pedicled flap of stomach tissue, which can improve outcomes of those patients.
