Study On The Effect Of XieHuoXiaoYing Decoction Combined With Methimazole In The Treatment Of Early Stage Hyperthyroidism With Heart And Liver Fire Flaming Syndrome

Objective:To investigate the clinical effects of XieHuoXiaoYing decoction combined with methimazole(MMI)in the treatment of Graves' disease in the early stage with heart and liver fire flaming syndrome.Methods:From July 2015 to October 2016,80 patients were enrolled in the study.All patients were from Nanjing University of Chinese Medicine Affiliated Hospital of integrated traditional Chinese and Western medicine.They were all meeting the diagnostic criteria of hyperthyroidism in the early stage and TCM syndrome of heart and liver fire flaming.According to random number table method,patients were randomly divided into control group and treatment group,and each contained 40 patients.The patients in the control group were treated with methimazole 10-30mg/d.The dosage gradually reduced to 2.5-10mg/d while symptom controlled.The patients in the treatment group were treated with methimazole combined with XieHuoXiaoYing decoction.Curative effects of two groups were evaluated after three courses(3 months)of treatment.Results:Total 61 patients finished this study.The heart rate,body weight,FT3,FT4,TSH and TRAb of two groups were comparable before treatment(p>0.05).The heart rate and body weight were both improved in the two groups after treatment.The heart rate in the treatment group was lower than the control group after treatment(65.00±12.00bpm vs.74.00±14.00bpm,p<0.01),while there was no significant difference in body weight between two groups.After treatment,FT3,FT4 and TRAb levels were significantly lower,TSH was significantly higher than before in both groups(p<0.01).After the first month of therapy,FT3 and FT4 decreased dramaticly in the treatment group(FT3 after the first month of therapy:5.56±2.46pmol/l vs.8.00 ± 3.95pmol/l,p=0.001;FT4 after the first month of therapy:19.90 ±8.28pmol/l vs.23.37±9.42pmol,p=0.019;FT3 after the second month of therapy:3.94±1.57pmol/l vs.4.961±2.187pmol/l,p=0.010;FT4 after the second month of therapy:13.52±4.69pmol/l vs.16±7.59pmol/l,p=0.043),while there were no significant difference between two groups after three months of therapy.After the first course of treatment,the recovery rate of FT3 and FT4 in the treatment group was higher than the control group(the recovery rate of FT3:19(63.33%)vs.11(35.48%),p=0.03;the recovery rate of FT4:21(70.00%)vs.12(38.70%),p=0.014).The average recovery time in the treatment group was shorter than the control group(the average recovery time of FT3:1.00±1.00month vs.2.00±2.00months,p=0.014;the average recovery time of FT4:1.00±1.OOmonth vs.2.00±1.00months,p=0.008).The initial dosage of methimazole between two groups of patients were no significant difference(30.0±15.0mg vs.30.0±15.0mg,p=0.620).During the three courses of treatment,the doseage of methimazole in the treatment group were all less than the control group(after the first month of therapy:15.00±6.25mg vs.20.00±15.00mg,p=0.30;after the second month of therapy:10.00±8.13mg vs.15.00±10.00mg,p=0.004;after the third month of therapy:5.00±5.75mg 10.00±10.00mg,p=0.015).TCM syndrome scores were decreased in both of the two groups(p<0.01),while much lower in the treatment group(5.38±4.00 vs.11.09±5.46,p<0.01).The adverse events in the treatment group were less than those in the control group,however,there was no significant difference.Conclusions:XieHuoXiaoYing Decoction combined with methimazole can increase efficiency in treating Graves diesease in the early stage meeting heart and liver fire flaming syndrome,which can decrease thyroid hormone levels faster,relieve the TCM syndrome,reduce MMI dosage,improve average recovery time and recovery rate.
