Study On The Community Management Of Diabetes Patients Based On The Grading Diagnosis And Treatment Policy

Objective:Based on the investigation of the current situation of community health service centers,the status of diabetes management,the status quo of diabetes and the cognition of patients in the new urban area of Urumqi,The existing problems and influencing factors are analyzed,To strengthen the community management of diabetic patients,On this basis,it is suggested to carry out the hospital community integrated diabetes management model,which can provide scientific basis for the implementation of the classification of chronic diseases in Xinjiang.Method:Under the background of the new national grading policy,A questionnaire survey was conducted on the basic situation of 12 community health service centers in Urumqi City,the situation of diabetes management,the situation of patients under the jurisdiction of the district and the knowledge of diabetes mellitus,Descriptive analysis was used to explain the basic situation of the 12 community health service centers,diabetes management and the situation of the patients in the area,F test and logistics regression were used to analyze and describe the knowledge of diabetes mellitus in patients with T.Results :(1)the basic situation of 12 community health service centers in Urumqi High-tech Zone: its coverage area population reached 325948,with a total area of 18018 square meters of real business,the total open 320 beds,the number of 435 workers.Workers in low,mainly in college and undergraduate,there were 195 people and 161 people,accounted for 44.8% of the total number of 37%;(2)diabetes management 12 community health service centers in Urumqi high-tech zones: as of 2015,high tech Zone 12 CHS the number of diabetic patients reached 34027;the number of 10717 people were filing,In addition to the individual CHS,the filing rate is generally low;community diabetes drugs with different uneven blood glucose control rate is low;2013-2015 high tech Zone 12 CHS for patients with diabetes health knowledge lectures accumulated 307 times,in 2013-2015 years of community health care professionals to carry out special training related to the number of diabetes accumulated a total of 71 times;(3)survey of the basic situation of patients: a total of 1023 patients were investigated,including a male of 431(42.13%),the Han nationality accounted for 80.55%,80.93% married,48.97% family income less than 50 thousand;(4)patients with diabetes knowledge awareness: the average knowledge score of the respondents was 7.437±0.285.Age,education level,marital status had an impact on the knowledge of patients with diabetes.Conclusion:The community diabetes management has the advantage of convenience and low cost,At the same time,there is a weak level of medical technology,the doctor's title,low academic qualifications,Older patients with lower education level have lower awareness of diabetes,In order to further improve the level of community diabetes management,based on the recommendations to strengthen the community diabetes management,Hospital community integrated diabetes management model.The establishment of this model will make up for the weakness of the medical system and the limited ability of the doctors at the basic level,To meet the needs of the community and the patients in improving the ability of diabetes management.
