Clinical Study On The Treatment Of Depression With Insomnia As Main Symptom By Jieyu 1 Decoction And Paroxetine

Background: With the process of urbanization,more and more people enter the faster pace of life,accompanied by the life and spiritual pressure,depression in this premise more frequent invasion of modern people.Depression is a mood disorder characterized by depression,a set of clinical syndromes.The main clinical symptoms of depressive episode include low mood,loss of interest and pleasure,insomnia,slow thinking,diminished mental activity and physical discomfort.According to the world health organization(WHO),depression will become the second disability disease after ischemic heart disease by 2020.The lifetime prevalence of depression in western countries and China is 13.23 % and 5.3 %,respectively.The female is at least 1.5 times of the male,a large number of literature reports show that the proportion is increasing in our country in recent years.In a state of depression patients,many patients with insomnia symptoms are particularly prominent.Long-term insomnia makes the psychological burden more serious,the other clinical symptoms of depression aggravate and increase.Other symptoms aggravated with increasing,in turn,further influence patients to sleep,eventually form a vicious circle.Objectives:(1)To observe the clinical effect of Jieyu 1 decoction combined with paroxetine in the treatment of depression with insomnia as main symptom.(2)To evaluate the feasibility of treating depression with traditional Chinese medicine combined with antidepressants,and to provide basis for the treatment of depression.Methods: 1.Group: Using random method of grouping,60 patients with depression as insomnia as main symptom were divided into study group 30 cases and control group 30 cases.The study group was treated by Jieyu 1 decoction combined with paroxetine hydrochloride,and the control group was treated with paroxetine hydrochloride alone.2.Indicators and curative effect: The treatment cycle is 8 weeks.Traditional Chinese medicine symptom integral,The Hamilton depression scale(HAMD)score,Pittsburgh sleep quality index(PQSI)score and the changes of safety indexes before and after treatment were measured before treatment and 4 weeks,8 weeks after treatment.Results:(1)Comparison of the same period before and after treatment,traditional Chinese medicine symptoms score,HAMD score and PSQI score of the treatment group and control group after treatment 4 weeks,8 weeks were significantly lower than before treatment.Compared with the treatment before treatment,it has a statistical significance(P<0.05 or P<0.01).(2)Compared with the two groups during the same period,the score of traditional Chinese medicine symptom score,HAMD score and PSQI score showed no significant difference before treatment(p > 0.05);Compared with the two groups in the same period,traditional Chinese medicine symptom score of treatment group after treatment 4 weeks than the control group was more obvious,there was significant difference between the two groups(p < 0.05),the HAMD score and PSQI score were less obvious than the control group(p > 0.05).Compared with the two groups during the same period,8 weeks after treatment,the treatment group was more obvious than the control group about the TCM symptom score,HAMD score and PSQI score,and there was significant difference between the two groups(p < 0.05).(3)Comparison of the same period before and after treatment,the factor points of TCM symptom score,HAMD score and PSQI score were improved in different degrees before and after treatment;In 8 weeks after treatment,the majority of factor points had significant difference compared with before treatment(p < 0.05 or p < 0.01);In 4 weeks after treatment,Partial factor points had significant difference compared with before treatment(p < 0.05);Overall,the treatment group was more obvious than the control group.(4)For the incidence of adverse drug reactions in two groups of patients,the treatment group than the control group is lower,the treatment group security is higher.Conclusion: For the treatment of depression patients with insomnia,Clinical effect by means of antidepressants as SSRIs can be obtained,when combined with Jieyu 1 decoction,the clinical effect is better and the incidence of side effects is reduced.
