Objective:Through the construction of acute septic shock rabbit model induced by hypertension with renal pelvis infection,the study simulates the process of acute septic shock after clinical lithotripsy,to study Salvianolic acid A(Sal.A),a kind of water-soluble phenolic compounds,which has a significant antioxidant effect,whether can improve the prognosis of acute infectious shock.Methods:Animal experiment:Firstly New Zealand rabbits were randomly divided into 4 groups,including group ABCD.Group A was injected saline to renal pelvis by 2ml/kg.Groups BCDE were injected E.coli 6.0*10^8cfu/ml,9.0*10^8cfu/ml,12.0*10^8.cfu/ml,respectively,to monitor the preoperative and postoperative mean arterial pressure(MAP).Secondly New Zealand rabbits were randomly divided into blank control group(E0),Standard shock model group(E1),Salvianolic acid A intervention group(E2),EO group was injected saline to renal pelvis by 2ml/kg,E1 and E2 groups were injected E.coli 12.0*10^8cfu/ml.0.5 hours after operation group E0 and E1 was injected by 3ml of physiological saline,group E2 was injected by 3ml Sal.A by 10mg/kg,monitor and record mean arterial pressure(MAP),Advanced oxidation protein(AOPP)and survival time.Results:Take MAP at 2 hours postoperatively as the basis for the MAP.Compared with basic MAP,MAP of Group A,B,C at 2 hours postoperatively had no significant difference.MAP in Group D at 3 h(68.8±9.2mmHg),4 h(64.6±7.9mmHg),5 h(64.6±7.9mmHg)postoperatively had no significant difference(P>0.05),however 6 h(68.8 ± 9.2mmHg)(P<0.05)and 8 h(68.8 ± 9.2mmHg)(P<0.01)had significant difference compared with basic MAP(69.0±8.8mmHg).We call the timing when The postoperative MAP became less than MAP as the shock time.Shock time of group D was 8.3±0.8 hours,and death time was 11.1 ± 1.1 hours.So we regarded Group D as the standard model of uroseptic shock induced by hypertension with renal pelvis infection.After operation,MAP of E0 group(blank control group)had no significant decrease or shock.The survival time was more than 24 hours.AOPP at 0,2,4,6 h postoperatively was basically stable.The MAP of the Group E1 at 6 hours postoperatively(61.6±10.3mmHg)decreased significantly.Postoperative shock of Group E1 occurred in 8.7 + 0.8 hours postoperatively,and death occurred in 11.1±0.9 hours postoperatively.AOPP of E1 decreased significantly at 6 and 8 hours postoperatively.The MAP of the Group E2 at 11 hours postoperatively(46.8±26.6mmHg)decreased significantly.Postoperative shock of Group E2 occurred in 11.9±1.1 hours postoperatively,and death occurred in 13.6±1.7 hours postoperatively.AOPP of E2 did not decrease significantly after the operation.Compared with Group E1,the shock time of Group E2 was significantly delayed(P<0.05),and also death time was significantly delayed(P<0.05),suggesting that the early antioxidant therapy of Sal.A can delay the occurrence of shock and death to some extent.The AOPP of group E2 decreased more quickly than E1 after 2 hours postoperatively.AOPP%,ratio of AOPP and preoperative AOPP,also proved this point.There was significant difference(P<0.05)between AOPP/Alb of the Group E1 and E2 after 2 hours postoperatively.Conclusion:From the MAP changes and survival curve,antioxidant effect of Sal.A could delay the shock and death time of shock modes,may improve the prognosis to a certain extent,but the effect is not very Significant.AOPP and AOPP/Alb prove that early intervention of Sal.A can reduce the oxidative stress level in the process of septic shock,reduce the oxidative damage.
