ObjectiveOsteoporosis is a common systemic bone disease in the elderly,characterized by decreased bone strength and increased risk of fragility fractures.The bone mineral density(BMD)and bone mineral content(BMC),are two important indicators reflect bone strength.Epidemiological survey shows,older men and women are reducing bone mass with increasing age,indicating that there will be more elderly people sick.BMD measured for the diagnosis of osteoporosis,prevention,assessment of concurrent fracture risk has important significance in this study,we will compare the difference BMD between lumbar spine and hip in both sexes,and discuss their value of primary osteoporosis diagnosis.MethodsSelect from June 2013 to June 2015 at Zhejiang University,Sir Run Run Shaw Hospital in elderly men and women,both in Zhejiang province a total of 300 residents were divided into two groups.103 cases are male,female 197 cases.Record patients basic information,L1 to L4 lumbar spine and hip BMD and the number of osteopenia and osteoporosis patients.Compare two groups of male and female parts of the BMD index level and relativity.Compare the incidence rate of osteopenia and osteoporosis patients.Compare the distribution of the incidence parts of osteoporosis.Results1.Each parts of Lumbar vertebra(L1?L2?L3?L4?L1-4)and hip(femoral neck?Wards area?Total hip)BMD in female group was significantly lower than it in male group(t=-9.42,-10.84,-10.66,-9.72,-11.05,-6.51,-8.95,-7.58,P ?<0.001).2.Between men and women lumbar vertebra BMD(L1-L4)were positively correlated(r=0.684?0.927,p<0.05)Each parts of hip BMD were positively correlated(r=0.825?0.848,p<0.05).Lumbar vertebra BMD(L1-L4)was positively correlated with the hip BMD(r-0.339?0.666,p<0.05)3.The osteoporosis checking rate of hip(20.4%)in male group was higher than the lumbar vertebra(12.6%).The osteoporosis checking rate of hip(46.7%)in female group was lower than the lumbar vertebra(56.9%).(p>0.05).4.Lumbar spine and hip joint inspection detection rates of osteoporosis in two groups were respectively 23.3%and 69.5%,Lumbar spine and hip joint inspection can improve both osteoporosis and osteopenia detection rate.5.Men and women with osteoporosis occur in different parts of the distribution percentage,in male group,only occur in lumbar vertebra was 12.5%,only occur in hip was 45.8%,occur in both parts was 41.7%.in female group,only occur in lumbar vertebra was 32.8%,only occur in hip was 18.2%,occur in both parts was 47.9%.(p<0.05)6.Men hip BMD osteoporosis diagnostic sensitivity is higher than lumbar BMD.Women hip BMD osteoporosis diagnosis sensitivity is lower than lumbar BMD.ConclusionThere is great value of lumbar vertebrae and hip bone mineral density in primary osteoporosis diagnosis.The older men can diagnose osteoporosis preferred measure hip BMD,women should also check the lumbar spine and hip BMD.If conditions permit,both men and women should also do the lumbar spine and hip bone density test to improve diagnostic accuracy.
