Clinical Observatiao On Shenfu Injection In Treating Septic Shock (Sudden Depletion Of Yang Qi Type)

Objective:Under the guidance the theory of Time Medicine for treatment,use the timing medication of shenfu injection to treat septic shock(sudden depletion of yang qi type),and to observe the clinical efficacy.Methods:Eighty-nine patients with ICU wards from the Second Affiliated Hospital of Harbin Medical University,diagnosed as septic shock(sudden depletion of yang qi type),were randomly divided into four groups by random table.There were the routine treatment group(n=23),and the observational treatment group,divided group noon,group midnight,group chenshi,(n=22).Group noon,on the basis of routine therapy,was treated with Shenfu Injection in the period of the day from 11 p.m to 1 a.m.Group midninght,on the basis of routine therapy,was treated with Shenfu injection in the period of the day from 11 a.m to 1 p.m.Group chenshi,on the basis of routine therapy,was treated with Shenfu Injection in the period of the day from 7 a.m to 9 a.m.After continuous treatment of 7 days,observed SOFA score(Sequential organ failure assessment)?Procalcitonin(PCT)?Cardiac troponin I(cTnI)measured by Automatic biochemical apparatus?Type B natriuretic petide(BNP)in blood?Mean arterial pressure(MAP)and Arterial blood lactic acid value(LAC)in patients of each group,were treated seventh days.Between the four groups,compared the observational datas in each two groups to determine the efficacy.Results:There were 89 patients in this study.During the observation,5 cases renounced treatment and transferred hospital;4 cases occurred severe infection and multiple organ failure to death.1.There were no significant difference in age,weight and APACHE ?score between four groups(P>0.05);2.The results of SOFA score showed that the other three groups were higher than the routine treatment group;group noon were higher than group midnight and group chenshi;group chenshi were higher than group midnight;the difference is statistically significant(P<0.05);3.The results of PCT score showed that the other three groups were higher than the routine treatment group;group noon were higher than group midnight and group chenshi;group chenshi were higher than group midnight;the difference is statistically significant(P<0.05);4.The results of BNP score showed that the other three groups were higher than the routine treatment group;group noon were higher than group midnight and group chenshi;group chenshi were higher than group midnight;the difference is statistically significant(P<0.05);5.The results of cTnI score showed that the other three groups were higher than the routine treatment group;group noon were higher than group midnight and group chenshi;group chenshi were higher than group midnight;the difference is statistically significant(P<0.05);6.The results of MAP score showed that the other three groups were higher than the routine treatment group;group noon were higher than group midnight and group chenshi;group chenshi were higher than group midnight;the difference is statistically significant(P<0.05);7.The results of LAC score showed that the other three groups were higher than the routine treatment group;group noon were higher than group midnight and group chenshi:group chenshi were higher than group midnight;the difference is statistically significant(P<0.05).Conclusion:1.There were curative effect,whatever the method of based on time medicine to take shenfu injection with routine therapy or simple routine therapy for septic shock patient(sudden depletion of yang qi type),but the "time" treatment was better.2.Taking medicine in the period of the day from 11 a.m to 1 p.m,curative effect was best.This may be related that the body was vital qi prevails over pathogens.
