| Objective:To Understand the osteoporosis distribution state of patients with type 2 diabetes,and its influence factors were analyzed,which may provide clinical clues for early prevention and treatment of diabetes mellitus complicated with osteoporosis.Methods:Clinical data was collected from 891 patients with type 2 diabetes who visited endocrinology department of 2nd Affiliated Hospital of Jilin University from September in 2014 to December in 2015,including 479 cases of males and 412 casess of females.The clinical and laboratory examination results(include blood lipid,blood pressure,fasting glucose,thyroid function,Hb A1 c and BMD)of these subjects were collected and analyzed retrospectively.Patients were classified into 3 groups according to BMD values:bone mass normal group(335cases),osteopenia group(399 cases)and osteoporosis group(157 cases).Compare the following risk factors of three groups:sex,age,waist circumference,body mass index,duration of diabetes,fasting glucose,smoking history,hypertension history,Fasting c-peptide,2 hours C peptide,BMD values,glycosylated hemoglobin,low-density lipoprotein cholesterol,triglycerides,total cholesteroland and the difference of thyroid function,and analyze the correlation between the above risk factors with BMD in T2 DM complicated by osteoporosis.Results:1)In terms of gender,in patients with type 2 diabetes and the proportion of women osteoporosis group was obviously higher than that of males,P < 0.0001.2)In terms of BMI level,BMI greater than or equal to 28 squared(kg/m2)in patients with type 2 diabetes,osteoporosis rate decreased significantly,P < 0.0001).3)Men is greater than or equal to 90 cm at waist level and women is greater than or equal to 85 cm in type 2 diabetes mellitus patients,reduced the proportion of osteoporosis,P = 0.0002.4)In older age,low waist circumference,low BMD,low triglyceride levels in patientswith type 2 diabetes,the proportion of osteoporosis were significantly increased,all P <0.0001;With high blood pressure,higher low density lipoprotein cholesterol levels in patients with type 2 diabetes,the osteoporosis proportion is higher,respectively,P=0.0121,P=0.0465;Osteoporosis group,glycosylated hemoglobin is higher,but not statistically significant(P = 0.7237);And smoking history,course of diseases,thyroid function,serum c-peptide levels in three kinds of bone mass group had no statistical significance of differences.5)By studying the potential risk factors and the correlation of different bone mass group concluded that in patients with type 2 diabetes,age,low density lipoprotein cholesterol(hdl-c)level and the positive correlation between osteoporosis;Waist circumference,fasting glucose,triglycerides,and femoral neck,lumbar 1-4,hip BMD values and negatively correlation with osteoporosis.Conclusion:1)Type 2 diabetes in age,sex,waist circumference,blood pressure,blood glucose,blood fat,body mass index and BMD values is closely related to the incidence of osteoporosis.2)Abdominal obesity and higher BMI level are protective factors for osteoporosis.3)Increased risk of osteoporosis in women,older age,low body mass in patients with Type 2 diabetes,who should pay attention to regular screening BMD values and at the same time to take early prevention of osteoporosis.4)In patients with type 2 diabetes,high blood pressure,blood lipid metabolic disorders,poor control of blood sugar can lead to increased risk of osteoporosis.So it is necessary to control your blood sugar stable at the same time also to maintain normal blood pressure and lipid levels. |
