ObjectiveBy summarizing and analyzing the clinical features of t he intrahepatic cholangiocarcinoma,and then compared with h epatocellular carcinoma to improve the understanding of ICC,provide more valuable information for clinic and enhance th e level of the diagnosis and treatment of l CC.MethodsWe collected 38 cases of ICC patients' data and selecte d 80 cases of HCC randomly in the same period to see a doct or in our hospital from January 2010 to December 2016.All patients undergoing surgery treatment and postoperative wer e confirmed by pathological diagnosis.Theclinical character istics,imaging gender,age,cause,manifestations,biochemical results,tumor markers,as well asimaging findings and patho logy features were restrospectively analyzed.ResultThrough the comparative analysis of the clinical data o f the ICC and HCC,we found that no obvious difference of gender was found between the two groups.The male proportion of HCC group was obviously higher than that of women,hower e,the rate of male patients was slightly higher than that of female patients in ICC group.The median age of ICC grou p was older than the control group.The ICC patients were mo re ofen with intrahepatic biliary calculi.But the proportio n of HBV infection,HCV infection and the patients with live r cirrhosis were lower than that of HCC group.The major cli nical manifestations of ICC and HCC group were abdominal p ain(44.74% VS 48.8%)?The positive rate and the detecti on value contrast of AST,ALT and TBIL in two groups were n o statistical difference.The positive rate and the detectio n value contrast of ALP and GGT in ICC group were higher th an that in HCC group.But the positive rate and the detectio n value contrast of AFP were lower in ICC group.The rate of AFP>400ng/ml in ICC group was significantly lower than th at in HCC group(2.6% VS 32.5%).The positive rate and the detection value contrast of CA19-9 were higher in ICC group.The rate of CEA>100U/ml in ICC group was significantly hig her than that in HCC group(52.6% VS 11.3%).Compared with the control group,the positive rate of CEA was no significa nt differences.Through iconography,we discovered that the ICC was more often in the left hepatic lobe and the tumor wa s mainly single.The ICC group also had more intrahepatic bi le duct expansion and abdominal lymph node enlargement.At t he pathology,most tumor of the two group was moderately and poorly differentiated.The rate of moderate and poor differ entiation in ICC was higher than that of high differentiati on(89.5% VS 10.5%).The ICC also had more local invasion a nd vascular invasion.Conclusion The ICC group compared with the HCC group:1.The mean age of ICC group was significantly older.2.The patients who found placeholder by medical examina tion were relatively more in ICC.3.The ALP and GGT of ICC group were more than that in H CC,therefore,they are helpful to diagnose the ICC.4.The patients who had high level CA19-9(especially >100U/ml)were more in ICC than that in HCC,but the patients who had high level AFP were less.5.The ICC was more often in the left hepatic lobe and t he tumor was mainly single,and the patients with intrahepat ic bile duct stones were more.6.The differentiation degree of ICC tissue is low.The malignant degree of ICC is high,and it often violates surr ounding tissues or organs.
