Objective:the levels of serum ?-glutamyl transpeptidase(?-GGT)and bilirubin(Bil)have been proved to reflect the level of oxidative stress.The purpose of this study was to investigate the expression of GGT and Bil in serum of patients with lupus nephritis((LN))and their relationship with the severity of lupus nephritis.Methods:1.All patients with LN treated in the affiliated Hospital of Southwest Medical University from July 2013 to August 2018.73 patients of whom followed urine protein to negative were selected as the LN1 group,91 patients with lupus nephritis with perfect 24-hour proteinuria(24H-Pro)and other auxiliary examination data were selected as the LN2 group,82 patients from the health examination center of our hospital in the same period were selected as the healthy control group.Exclusion criteria:(1)patients with transaminase and bilirubin exceeding the clinical reference range;(2)patients with renal decompensation whose serum creatinine increased more than 177umol/L;(3)patients with primary cardio-cerebrovascular disease and liver disease;(4)patients with malignant tumor;(5)patients with systemic lupus erythematosus complicated with other severe organ involvement;(6)patients whose urine routine leukocyte and erythrocyte abnormal;(7)patients with imperfect clinical data.2.Basic demographic data as sex and age of eligible patients were collected,auxiliary examination data as 24-hour proteinuria(24H-Pro),?-glutamyl transpeptidase(?-GGT),total bilirubin(Tbil),direct bilirubin(Dbil),indirect bilirubin(Ibil),complement 3(C3),complement 4(C4)and creatinine(Cr)were collected.3.Chi-square test was used to compare the gender composition of healthy control group and LN1 group.Independent sample t-test was used to compare the differences of GGT and BIL groups between healthy group and LN1 group before treatment.Paired sample t-test was used to compare the levels of GGT and BIL before and after treatment in LN1 group.Pearson correlation analysis was used to test the correlation between GGT and Bil and clinical indexes such as 24hU-Pro,C3,C4,Cr,ALB and so on.SPSS17.0 statistical software was used.P < 0.05 has statistical significance.Results: 1.The level of GGT in patients with LN before treatment was significantly higher than that in healthy controls [(26.026 ±16.135)U/L,(14.840 ±11.086)U / L,P<0.05],and the level of GGT in patients with LN after treatment was significantly lower than that before treatment [(17.999 ±12.0939)U/L,(26.026 ±16.135)U/L,P<0.05].The level of serum Tbil in patients with LN was significantly lower than that in healthy controls [(9.298 ± 3.576)umol/L,(13.010 ± 4.216)umol/L,P<0.05],and the level of serum Dbil in patients with LN was significantly lower than that in healthy controls [(2.868 ± 1.371)umol/L,(4.416±1.469)umol/L,P< 0.05].The level of serum Ibil in patients with LN was significantly lower than that in healthy controls(6.241±2.600)umol/L,(8.594±2.940)umol/L,P<0.05],the levels of Tbil,Dbil and Ibil in patients with LN had no significant change after treatment.2.Pearson correlation analysis confirmed that there was a correlation between serum GGT level and 24H-Pro in patients with LN(r=0.411,P < 0.05).The level of serum Dbil in patients with LN was correlated with 24H-Pro,and the level of GGT was correlated with the level of serum creatinine,but not with C3,C4,ALB,Tbil,Dbil,Ibil,.Tbil,Dbil,Ibil were all correlated with ALB,but not with C3,C4,Cr.Tbil and Ibil were not correlated with 24H-Pro.Conclusion:1.The level of serum GGT and Bil may be related to oxidative stress in patients with LN,resulting in significant statistical differences in the level of GGT and Bil between patients with LN and healthy subjects.3.The levels of serum GGT and Dbil are correlated with 24H-Pro in patients with LN,therefore,the levels of serum GGT and Dbil can indirectly reflect the renal involvement of patients with LN,and the condition and therapeutic effect of LN patients may be evaluated indirectly by monitoring the level of serum GGT and Dbil.
