Objective:To investigate whether NLRP3 and correlation factors can be used to predict the instability of coronary atherosclerotic plaque and left ventricular dysfunction in patients with coronary heart disease, through detecting the differences of levels of NLRP3 and correlation factors IL-1?,IL-18 in peripheral blood of patients with acute myocardial infarction, unstable angina pectoris and stable angina pectoris, and their correlation with C reactive protein(CRP),white blood cell count(WBC)and left ventricular ejection fraction(LVEF).Method:80 patients with chest pain in Fourth Affiliated Hospital of Guangxi Medical University(Liuzhou Worker's Hospital)from March 2015 to August 2015 were selected. They were divided according to the diagnostic criteria of coronary heart disease.Among them, there were 26 cases of stable angina pectoris(SAP group),28 cases of unstable angina pectoris(UAP group),and 26 cases of acute myocardial infarction(AMI group). All patients underwent coronary angiography and they were diagnosed as coronary heart disease. At the same' time,24 patients with normal coronary angiography were enrolled as control group(NC group).Enzyme linked immunosorbent assay(ELISA)was used to detect the levels of NLRP3,IL-1? and IL-18 in serum of each group. The correlation between NLRP3,IL-1? beta,IL-18 and CRP,white blood cell count and left ventricular ejection fraction was analyzed by Pearson correlation analysis. Results:The expression levels of NLRP3 and IL-1? in peripheral blood of patients with coronary heart disease were significantly higher than those in control group. The levels of NLRP3,IL-1? and IL-18 protein in AMI group were significantly higher than those in UAP group,SAP group and NC group(P<0.05).The levels of NLRP3 and IL-1? beta protein in UAP group and SAP group were significantly higher than those in group NC(P<0.05).The level of NLRP3 protein in UAP group was significantly higher than that in SAP group(F=5.071,P=0.029).The levels of plasma NLRP3,IL-1? and IL-18 protein were positively correlated with white blood cell count(WBC)and C reactive protein(CRP)in patients with coronary heart disease, and negatively correlated with left ventricular ejection fraction. Conclusion:In patients with coronary heart disease, the plasma NLRP3 protein levels in AMI and UAP group were significantly higher than those in SAP group, which suggests that the high expression of NLRP3 may be related to the instability of coronary atherosclerotic plaques in patients with coronary artery disease. The levels of plasma NLRP3,IL-1? and IL-18 protein were negatively correlated with left ventricular ejection fraction(LVEF)in patients with coronary heart disease.
