| Objective:To study the correlation on the preoperative serum squamous cell carcinoma antigen SCCA with clinical biological characteristics of early stage cervical squamous cell cancinoma and investigate the correlation between SCCA and 3-year,5-year survival rate,disease-free survival time(DFS)and total survival time(OS).To reveal the relationship between preoperative SCCA level and the clinical biological characteristics of patients with early cervical squamous cell carcinoma,and to analyze the relationship between preoperative SCCA level and prognosis of early cervical squamous cell carcinoma.Methods: 197 patients with stage Ia~IIa cervical squamous cell carcinoma was collected from Yijishan Hospital,affiliated to Wannan Medical College from January 2008 to December 2011.And all these patients were treated with primary surgery.Retrospectively analyze the correlation of preoperative serum level of squamous cell carcinoma antigen(SCCA)and the age,clinical stage,lymph node metastasis,tumor diameter,depth of myometrial invasion,vascular metastasis in patients with early cervical squamous cell carcinoma.All patients were followed up for SCCA,the level of preoperative SCCA and postoperative SCCA were compared and analyzed to evaluation of therapeutic effect;And the survival of the negative group and positive group after operation were compared and analyzed.The 3 year and the 5 year survival rate,tumor free survival time(DFS)and overall survival time(OS)of thepreoperative SCCA positive and negative groups were statistically analyzed respectively.Results: The expression of SCCA before operation was closely related to the clinical stage,lymph node metastasis,focal lesion diameter,depth of cervical myometrial invasion and vascular invasion(P <0.05),and there was no significant difference with pathologic grade and age.Preoperative serum SCCA level in 187 patients was(3.37 ±5.78)ng/ml and three month after operation,the serum SCCA level was(1.42±2.15)ng/ml,the difference was statistically significant(P<0.05).In 49 cases of preoperative SCCA positive group,the Overall survival time(OS)of the postoperative SCCA level decreased to normal patients(n=43)was better than that of abnormal patients(n=6)(P<0.05).In the preoperative SCCA negative group,the disease-free survival time and the 3 year and the 5 year disease-free survival rate were significantly better than those in the preoperative positive group(P<0.05).There was no significant difference in the 3 year survival rate of preoperative SCCA negative group and SCCA positive group(P>0.05).There was no significant difference in the 3 year survival of positive SCCA patients with patients with negative SCCA before operation(P>0.05).But as time went on,the overall survival time(OS)of SCCA in patients with negative SCCA were longer than those of positive SCCA(P<0.05).Moreover,the total survival time(OS)of patients with preoperative positive SCCA was significantly lower than that in the preoperative negative SCCA group.Conclusion: Squamous cell carcinoma antigen as a tumor marker is important for the evaluation of the severity and progression of cervical squamous cell carcinoma and it also can be used as an indicator of therapeutic efficacy.SCCA plays an important role in predicting the prognosis of patients with early cervical carcinoma and its follow-up.The positive rate of SCCA in early cervical squamous cell carcinoma is not very high,still can not be used as a diagnostic criteria for cervical squamous cell carcinoma,can only be used as an auxiliary diagnostic factors. |
