The Application Of Enhanced Recovery After Surgery Combined With Laparoscopy-assisted Distal Gastrectomy For Gastric Cancer

Objective:To compare the clinical outcomes in gastric cancer patients with fast tract surgery and conventional treatment.Methods:Eighty-four patients were admitted to the first affiliated hospital of nanchang university with distal gastric cancer from January 2015 to November 2015,and were randomly divided into four groups:(1)FTS+LADG(n=21),treated with LADG and FTS treatments;(2)LADG(n=21),treated with LADG and traditional treatments;(3)FTS+ODG(n=21),treated with ODG and FTS treatments;(4)ODG(n=21),treated with ODG traditional treatments.The age,sex,body weight,anastomotic mode,number of lymph node dissected,and tumor stage,serum albumin(ALB),C-reaction protein(CRP),flatus time,postoperative hospital stay,medical cost,and postoperative complications were compared between four groups.Results:1.To compare FTS group with conventional treatment group,the FTS group of recovery time more earlier than the conventional treatment group,hospitalization time more shorer than conventional treatment group,The average hospitalization expense of four groups,FTS+LADG group lower than LADG group,but still significantly higher than the FTS + ODG and ODG group,FTS + ODG group is the Lowest.2.To compare the Laparoscopic surgery group(FTS + LADG)with LADG group and open surgery group(FTS + ODG and ODG group),The Laparoscopic surgery group of total blood loss decreased significantly,but the operation time obviously prolonged.3.compared with conventional treatment group,the level of ALB in the FTS groups was higher,the level of CRP recovery faster,Lower stress reaction and better postoperative analgesic effect,but to the postoperative complications,there were no differences between the four groups.Conclusions:The combination of FTS and laparoscopic resection for distal gastric cancer is safe and feasible.it can accelerate gastrointestinal peristalsis,shorten hospital stay,reduce the stress response,the combination of FTS and LADG has some advantages in the clinical application.
