Background:Due to long-term pressure on the skin, the degradation of organs' functions and decreased nutritional status, bedridden patients have high risk in the occurrence of pressure ulcer(PU).PU is a worldwide health problem, which increases mortality, prolongs hospital stay, reduces patients' living quality, stays maintains a high incidence. With the acceleration of aging process, prolonged survival of chronic diseases, more and more scholars pay attention to exploring the intervention of PU in bedridden patients. At present, most domestic and foreign studys focus on exploring the PU's characteristics in bedridden patients with special diseases, which can't describe the overall situation of PU in bedridden patients. Therefore,it's necessary to conduct epidemiological surveys to explore the prevalence, incidence, the nursing prevention and influential factors of PU among bedridden patients in China's general medical facilities, which is helpful to decrease the prevenlence of PU and improving nurses' preventation ability and bedridden patients' living quality. Objective:To describe the PU prevalence, incidence and the effectiveness of PU prevention in bedridden patients; To analyze the influential factors of PU.Method:It's a descriptive study. The self-designed Case Report Form were developed which contains demographic information, physical assessments' results, treatment information, the description of PU and nursing prevention and intervention. In this study, using rate to describe the PU prevalence, the PU incidence and the nursing prevention, using Chi-square test and multivariate logistic regression analysis to explore the influential factors.Results:(1)The overall PU prevalence with(without)stage 1 is 3.38%(1.68%),the PU incidence with(without)stage 1 is 1.26%(0.67%).(2)Age,gender,innutrition,fracture of lower limb, spinal cord injury, the hospital level, the treatment experience in ICU,the length of bed days, the revised Braden scale score, low hemoglobin, urinary incontinence, the use of plaster or brace, the use of enteral nutrition support therapy, the use of vasoconstrictor drugs are the main influential factors of bedridden patients.(3)The Sensitivity of the revised Braden scale is 0.72,the Specificity is 0.61,the Positive Predictive Value is 23.05%,Negative Predictive Value is 99.42%and the AUC is 0.72.(4)Among the samples with the revised Braden scale score<19,91.14%acquired the standard repositioning measures, the partial decompression tools can cover 74.60%bedridden patients. Conclusions:The PU prevalence and incidence of bedridden patients in general hospitals are higher than other domestic studys, and PU prevalence and incidence with(without)stage 1 is lower than domestic studys among special bedridden patients. The general information, laboratory test indicators, treatment information can affect the risk of PU in bedridden patients, which reminders nurses should pay attention to evaluating these influencial fators and take the preventive measures timely. The reviesed Braden scale is needed to be improved among bedridden patients. The PU nursing preventive measures about bedridden patients in this survey is in good condition. In the future, researchers should pay attention to establishing a standardized PU preventive intervention model in bedridden patients to reduce the incidence of PU and improve the living quality of bedridden patients.
