Objective:To summed up the policy design ideas,the main practices and issues of the establishment record system reform in the typical public hospitals;and understand the views of medical staff on the establishment record system reform;and compare medical staff satisfaction of different ways of establishment management,finally provide a basis for decision-making for comprehensively implementing the establishment record system in our country.Contents:1.Review the concept and theory of establishment system reform on public hospital in our country.2.Analyze the specific practices of the establishment system reform of the public hospitals in the research area.3.Analyze and compare the medical personnel satisfaction of different ways of establishment management after the preparation of the establishment system reform.4.Provide the policy recommendations related to improving the establishment system reform on public hospital and to mobilizing the enthusiasm of medical staff.Methods:In this study,we use literature review and qualitative interview method to sort out the relevant theories,policies and research literatures,and construct the theoretical analysis framework of the impact of the management style on the medical personnel satisfaction.The questionnaire was used to collect the medical personnel satisfaction data,and the descriptive analysis and the single factor analysis were used to compare the satisfaction degree of the medical staff with different establishment management methods.After stratified by occupation and professional title,the satisfaction degree of medical staff in different forms of establishment management was analyzed and compared.Multivariate logistic regression analysis was used to control confounding factors,and the effects of different establishment management methods on the satisfaction of medical staff were analyzed.Result:1.The overall satisfaction rate of medical staff was 56.5%.2.There were significant differences in the satisfaction degree of the overall satisfaction,the type of self,the salary level,the total income,the old-age insurance and the post evaluation condition of the different establishment management methods(P<0.05).The satisfaction rate of the medical staff for establishment record system in the post is higher than that of the medical staff for establishment approval system,and other aspects are lower than those of the medical staff for establishment approval system.3.There were significant differences in the satisfaction degree of the overall satisfaction,the salary level,the old-age insurance and the post evaluation condition of the nursing staff for different establishment management methods after stratified by occupation(P<0.05).The overall satisfaction and job satisfaction of the establishment record system nursing staff were higher than those of the establishment approved nursing staff.But the satisfaction of salary level and the old-age insurance were lower than those of the establishment approved nursing staff.There were significant differences in the satisfaction of salary level of medical staff and administrative staff for different establishment management forms(P<0.05).And the satisfaction of salary level of medical staff and administrative staff for establishment record system was higher than those of the establishment approved staff.Different establishment management methods of physician in the job evaluation of the satisfaction of the difference was statistically significant(P<0.05).Establishment record system personnel satisfaction was higher than the establishment approval system staff.There were significant differences in the satisfaction degree of the overall satisfaction,the salary level,the old-age insurance and the training and the job-job matching of the primary title personnel for different establishment management methods after stratified by title(P<0.05).The staff satisfaction of these o ? the approval system were higher than the establishment record system personnel.There were significant differences in the satisfaction of salary level of the mid-level personnel for different establishment management methods(P<0.05).The satisfaction on salary level on supernumerary staff was higher than the approval system and record system personnel.4.After the control of other variables,the finding of orderly multivariate logistic regression showed that compared with the supernumerary staff,the approval system and the record system could improve the overall satisfaction of the medical staff and the differences have a greater impact on the satisfaction of the record medical staff.In terms of the satisfaction of the establishment reform,the possibility of reducing the satisfaction level of the one or more grades of the approval system and the record system of the medical staff on the reform policy design was significantly reduced by 32.5%and 56.7%respectively.The satisfaction of the establishment reform more greatly impact on the record medical staff.In terms of the satisfaction of the salary and pension,the approval system and record system can improve the satisfaction of medical staff for salary and pension,but the reform of salary and pension have greater impact on the satisfaction of medical personnel for the record system.Conclusion:1.The overall satisfaction of medical staff in public hospitals in the sample area was at a moderate level.2.In general,the reform of the public hospital establishment system has a greater impact on both the overall satisfaction and the satisfaction of the the reform,salary and pension,career development and job placement of the medical staff on the record system.Nearly 50%of the medical staff argued that the record system was more suitable for the current development of public hospitals.3.There were most obviously significant differences in the satisfaction degree of the salary level and the old-age insurance of the medical staff on different establishment management methods.Salary and welfare and pension insurance were still improved in the follow-up reform policy.Recommendation:Government level:At first,central government should strengthen the top-level design of the establishment record system reform and establish linkage mechanism of relevant departments.Secondly,the supporting policies related to establishment record system reform should be perfected.Thirdly,in order to keep abreast of the medical staff to reflect and satisfaction,the relevant departments should be tracked to monitor.Medical institutional level:Firstly,the hospital should improve the management of record system personnel and strive to heighten their social insurance benefits.Secondly,the professional development and personal growth of medical staff should be focused by hospital.Individual and social level:First of all,the medical personnel should adapt to the reform policy of the establishment record system and strengthen their self-culti vation.Secondly,as a reform advocator,the media should do a good job on establishment record system reform report in public hospital.
