Objective: To explore the correlation of blood pressure(BP)response in exercise testing in patients with subclinical target organ damage and expression level and clinical significance of microRNAs in hypertension patients in plasma and peripheral blood mononuclear cells(PBMC).Methods: A total of 90 hypertension patients were divided,according to levels of blood pressure in exercise testing,into common hypertension group(n=55)and exercise hypertension group(n=35),selected 20 Normal healthy people as normal control group.Target organ damage related indicators(LVMI,IMT,eGFR,mAlb)were recorded.Expression of microRNAs in plasma and PBMC in different groups was detected by real-time quantitative polymerase chain reaction(PCR).Results:systolic and diastolic blood pressure of the moment of maximum load movement and 2 minutes after exercise were significantly higher than the moment of resting in three groups(P < 0.05).systolic and diastolic blood pressure of the moment of 6 and 10 minutes after exercise were lower than the moment of maximum load movement(P<0.05).Change ranges of systolicblood pressure is more obvious than diastolic pressure.Exercise hypertension group were significantly higher than common hypertension group and normal control group in target organ damage related indicators(LVMI,IMT,eGFR,mAlb)(P<0.05);The plasma expression of microRNA-145 of common hypertension group and exercise hypertension group is significantly lower than normal control group(P<0.05),and exercise hypertension group is lower than common hypertension group in expression of microRNA-145 in plasma(P<0.05);There was no significant difference in expression of microRNA-143 in plasma between 3 groups(P>0.05),The plasma expression of microRNA-21 in common hypertension group was respectively higher than normal control group and exercise hypertension group(P<0.05);Compared with normal control group,the PBMC expression of microRNA-145 and microRNA-143 in common hypertension group and exercise hypertension group significantly decreased(P<0.05).The PBMC expression of microRNA-145 and microRNA-143 in exercise hypertension group was lower than common hypertension group,The PBMC expression of microRNA-21 in exercise hypertension group was higher than normal control group,but lower than common hypertension group(P<0.05).Conclusion: exercise hypertension can be regarded as an important factor in assessing the relationship between target organ damage related indicators and target.It is discovered that microRNAs have different expression in plasma and PBMCs of patients with normal hypertension and exercise hypertension.
