Objective:Through statistical analysis of patients with bladder cancer after TURBT,the pathological diagnosis of MIBC and refused to treat the effect and prognosis of RC,provide a new way for the diagnosis and treatment of MIBC patients.Methods: Through a retrospective analysis of our hospital from January 2011 to December 2015 after TURBT,the pathological diagnosis of MIBC,and refused to RC patients in 110 cases.There were 79 males and females,aged 38-89 years,with an average age of 67.3 years.During the operation,the tumor was resected completely,and the depth of the outer wall of the bladder wall was beyond the level of 0.5-1.0cm.105 patients were treated with intravesical instillation after operation,10 patients were treated with systemic adjuvant chemotherapy with GC protocol for 4 cycles,5 cases underwent internal iliac arterial chemoembolization,another 5 cases lost follow-up after operation.Results: In this group of 110 patients with TURBT,the operation was successful,the average operation time was 43 min,the average intraoperative blood loss was 15 ml,no serious complications occurred in all cases;The average length of hospital stay was 7.7 days.All 105 patients were followed up successfully.Among them,1 0cases were treated with systemic chemotherapy of GC cycle of 4 cycles.5 casesunderwent internal iliac artery embolization chemotherapy.Postoperative recurrence occurred in 28 cases,with a recurrence rate of 26.7%,The average recurrence time was 20.6 months(1-62 months).18 cases were treated with TURBT after recurrence,and partial cystectomy with GC was performed in the treatment of pelvic radiotherapy in the treatment of bladder cancer in 1cases,and Partiall cystectomy was performed in 7cases and conservative treatment in 2 cases.There were 78 patients with follow-up time of less than 2 years,with a recurrence rate of 26.9% and an average recurrence time of 25 months.There were 102 cases of survival,and the death of 3 patients,of whom,1 died of recurrence and metastasis of bladder cancer,1 patients died of advanced gastric cancer metastasis,and the other 1 patients died at the end of the operation for 12 months,the cause is unknown.Conclusion:For the refusal of radical cystectomy in all patients with invasive bladder cancer,TURBT is an effective treatment.This surgical trauma is small,quick recovery,fewer complications,the quality of life after surgery is guaranteed.For patients with bladder preservation,the need for regular review after TURBT combined with bladder perfusion and adjuvant chemotherapy,can achieve an ideal effect.
