The Double Eyelid Surgery Combined Inverted L Method And Park "Z" Method Correcting Epicanthus Clinical Analysis

Objective:Through the double-eyelid operation of the incision method, the clinical effect of the inverted L method and the Park "Z" method for correcting epicanthus were analyzed and compared, and the advantages and disadvantages were discussed.Methods: September 2014 to September 2016 period, in the Second Affiliated Hospital of Kunming Medical University plastic surgery department due to double eyelid eyelid surgery in patients with 130 cases. Epicanthus according to severity divided into mild, moderate and severe, corresponding groups were divided into Group A (n = 50), Group B (n = 30) and Group C (n = 50). Then, Group A can be randomly dividied into group a1 which was using the inverted L epicanthoplasty and group a2 which the Park "Z" method is adopted. Similarly, the Group B was divided into group bi and group b2;Group C into group ci and group c2 .The changes of ocular morphology were observed at 3 months after operation.The clinical efficacy was evaluated by measuring the intercanthal distance (ICD) with preoperative and postoperative 3 months and the Patient and Observer Scar Assessment Scale(POSAS).Results: After a three-month follow-up, 128 patients completed follow-up. After postoperative 3 months the comparison result is: (1) the ICD shortening rate: The patients’ ICD shortening rate between group ai and group a2 has no statistical significance(P=0.211).Group a, got(13.67%± 0.328) and group a2 got(12.82% ± 0.38)respectively.The patients’ ICD shortening rate between group bi and group b2 has no statistical significance(P=0.065). Group b1 got(14.77% ± 0.266) and group b2 got(14.23% ±0.327) respectively. The patients’ ICD shortening rate between group ci and group c2 has no statistical significance(P=0.234 ).Group c1 got( 15.13% ±0.255 )and group c2 got( 14.33% ± 0.31) respectively.(2) patients with scar overall satisfaction rating:The overall patients’ satisfaction between group ai and group a2 has no statistical significance(P=0.0.728).Group ai got(3.71 ± 1.601) scores and group a2 got(4.25 ± 1.595)scores respectively.The overall patients’ satisfaction between group bi and group b2 has statistical significance(P=0.0.008). Group bi got(3.13 ±1.187)scores and group b2 got(4.20±2.111)scores respectively. The overall patients’satisfaction between group ci and group c2 has statistical significance(P=0.010).Group c1 got(4.00 ± 1.291)scores and group c2 got( 5.56 ± 1.805) scores respectively.Other differences scar assessment scales (including PSAS score and OSAS score) also support the above argument.Conclusion: For mild epicanthus, two kinds of operation of postoperative ICD shorten rate have no difference. The postoperative scar was not obvious. The effect was satisfactory. we can choose inverted L method or Park "Z" method to correct the inner canthus skin. But for moderate and severe epicanthus, we tend to choose the inverted L method to correct the inner canthus skin,because the inverted L method can be better than the Park method of postoperative satisfaction, the former postoperative scar was significantly lighter than the latter . The two kinds of surgery can exposure the canthus angle, and the thick eyelid line convergence of natural smooth, can be used as a correction of single eyelid combined with epicanthus is ideal for surgery.
