The difference of microcirculation between preoperative and postoperative femoral head necrosis was analyzed by dynamic contrast-enhanced MRI(DCE-MRI)scan to explore the pathogenesis of early femoral head necrosis.To analyze the early and mid-term clinical effects of core decompression and negative pressure attraction in the treatment of early femoral head necrosis,and to provide a more effective theoretical basis and treatment for the clinical treatment of early femoral head necrosis.Medullary decompression combined with negative pressure aspiration in patients with early femoral head necrosis who met the inclusion criteria,The patients were followed up for 1,3,6,12,18 and 24 months.The VAS score and Harris hip score were recorded,and parallel hip orthotopic X-ray examination,In order to determine the recovery of hip function at different times after operation;The Ktrans,Ve,Kep and iAUC values of the vascular endothelial dysfunction were detected by dynamic MRI scan of the hip in the patients with necrosis of the femoral head during the follow-up period,further quantitative analysis of early femoral head necrosis in the preoperative and postoperative microcirculation changes in different periods.All patients were not found to have necrosis of the femoral head after the collapse of the morphological changes,necrotic femoral head lesions no significant expansion of the disease has been effectively controlled.The difference of Harris score and VAS score between the two groups was statistically significant(P <0.05).Imaging data and Tissue 4D software processing data for preoperative and postoperative comparison,In the ARCOI necrosis group,the postoperative microcirculation parameters of the Ve value of 1,3 months compared with the preoperative difference between the P> 0.05,no statistically significant;The difference of Kep between the 1 month and the preoperative rate was no significant difference between the two groups(P> 0.05);There were significant differences in microcirculation parameters between the other time and the preoperative(P <0.05).This study shows that medullary decompression combined with negative pressure aspiration is a new treatment of early femoral head necrosis of the Baotou operation,the new surgical treatment of early femoral head necrosis in the early and mid-term clinical efficacy of significant results,patients with postoperative hip function,pain symptoms have been effectively improved,and delay and even control the rapid development of the femoral head necrosis,to prevent the collapse of necrotic femoral head role.The results showed that the new surgical method had a certain effect on the treatment of early femoral head necrosis,promoted the early femoral head necrosis and capillary generation in the border area,and improved the microcirculation of the necrotic femoral head.In this study,we use the pre-study as the theoretical basis,the core decompression combined with negative pressure to attract the clinical treatment of early femoral head necrosis,to explore the surgical treatment of early femoral head necrosis of the clinical efficacy.Due to the limitations of the number of study samples,the new technique can lead to more accurate conclusions on the microcirculation changes and clinical efficacy of early femoral head necrosis,such as follow-up of long-term cases,pathological experiments and the increase of sample size.
