The Clinical Research Of Evaluating The Treatment Of Detoxification Washing Medicine In Chronic Venous Insufficiency With Lipodermatosclerosis By Using Doppler

Objective: Evaluating the curative effect of detoxification washing medicine in the treatment of LDS(Lipodermatosclerosis,LDS).Methods: Screening 68 cases of patients with typical LDS who were treated during March 2016 to December 2016,were randomly divided into experimental group with 38 cases and control group with 30 cases,respectively with the application of detoxification washing medicine and isothermal amount of clear water,cooperated with the foundation treatment for 28±2 days in the affiliated hospital of shandong university of TCM.Recorded clinical symptoms and signs before and after the treatment,including swell,pain,skin color,acid bilges and fatigue;Recorded color doppler ultrasound imaging features before and after the treatment with the application of color doppler ultrasound technology and digital skin temperature and resistance meter device,exploring the changes of imaging and the skin temperature in the lesion area,including the structure and thickness of injured subcutaneous tissue;Pathological changes of subcutaneous tissue elasticity index;Hemodynamic characteristics of the limb venous and the function index of venous valve.According to quantitative assessment,calculating the decrement of integral limb symptoms and the improvement of skin temperature and thickness and elasticity index of skin fascia before and after the treatment according to the quantitative assessment.Results: 1.With the comparison of skin temperature,the thickness of the superficial fascia,the thickness of the deep fascia,fascia elastic index before the treatment of humid and hot type,blood stasis type and spleen kidney Yang deficiency type,there are statistical significance(P<0.05).2.Clinical symptoms integral,experimental group and control group before treatment had no significant difference(P>0.05),and had significant different after treatment(P<0.05).21 cases were clinical cured in experimental group,10 cases were markedly improved,4 cases were effective,3 cases were invalid,the effective rate was 92.1%.13 cases were clinical cured in control group,6 cases were markedly improved,4 cases were effective,7 cases were invalid,the effective rate was 77%.The skin temperature,experimental group and control group before treatment had no significant difference(P>0.05),and had significant different after treatment(P<0.05).There are 37 effectively cases and 1 ineffective case in the experimental group;There are 28 effective cases and 2 ineffective cases in the control group.Thickness of the superficial fascia,experimental group and control group before treatment had no significant difference(P>0.05),and had significant different after treatment(P<0.05).There are 36 effectively cases and 2 ineffective case in the experimental group.There are 25 effective cases and 5 ineffective cases in the control group;The thickness of the deep fascia,experimental group and control group before treatment had no significant difference(P>0.05),and had no significant different after treatment(P>0.05).There are 28 effectively cases and 10 ineffective case in the experimental group;There are 22 effective cases and 8 ineffective cases in the control group.The fascia elasticity index,experimental group and control group before treatment had no significant difference(P>0.05),and had no significant different after treatment(P>0.05)There are 33 effectively cases and 5 ineffective cases in the experimental group;There are 24 effective cases and 4 ineffective cases in the control group.Conclusion: 1.The skin temperature of three syndrome types from high to low in turn is humid and hot type,blood stasis type and spleen kidney Yang deficiency type;The superficial fascia skin thickness from high to low in turn is humid and hot type,blood stasis type and spleen kidney Yang deficiency type;The skin deep fascia thickness from high to low in turn is spleen and kidney Yang deficiency type,blood stasis type and humid and hot type;The fascia skin elasticity index of three syndrome types from high to low in turn is spleen and kidney Yang deficiency,blood stasis type and humid and hot type.The implicit femoral venous valvular regurgitation degree from high to low in turn is spleen and kidney Yang deficiency,blood stasis type and humid and hot type.The degree of disordered skin structure from high to low in turn is spleen and kidney Yang deficiency,blood stasis type and humid and hot type.2.The detoxification washing medicine has best effect which can obviously relieve the clinical symptoms of patients of the CVI with typical LDS.3.The detoxification washing medicine can obviously decrease the lesion skin temperature;The detoxification washing medicine can decrease the thickness of the skin superficial fascia;The effect of detoxification washing medicine with the treatment of deep fascia and fascia elasticity index is not obvious.
