Objective: It is to analyze the relationship between serum leptin(LP),soluble leptin receptor(SLR)and silent information regulator factor 2 related enzyme 3(SIRT3)with hypertensive left ventricular hypertrophy(HLVH)and atherosclerosis(AS)in hypertension.Methods: 60 cases with essential hypertension(EH),who were divided into the patients with hypertension stage 1,stage 2 and stage 3 according the hypertension grading,were selected as the EH group,and 60 cases of healthy adults were selected as the control group.All the subjects did UCG for left ventricular mass index(LVMI)and carotid intima media thickness(cIMT),and all subjects were divided into hypertensive left ventricular hypertrophy group(HLVH group,n=33),essential hypertension without LVH group(non-LVH group,n=27),healthy subjects(control group,n=60)and cIMT normal group with 55 cases,cIMT thickness group with 52 cases and the plaque formation group with 13 cases based on LVMI and cIMT,respectively.Serum LP,SLR and SIRT3 levels were tested by enzyme-linked immunosorbent assay(ELISA)method,LVMI and cIMT was used to measure the degrees of LVH and AS,examining the relative indicators of fasting plasma glucose(FPG),total cholesterol(TC),triacylglycerol(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),blood urea nitrogen(BUN)and creatinine(CR),etc.Results: 1.The LVMI,cIMT and serum LP,SIRT3 levels of EH group were significantly higher than those of the control group,while serum SLR level of EH group was significantly lower than that of control group,and differences were statistically significant(P<0.05).2.Differences among LVMI,cIMT and serum LP,SIRT3 levels of the patients with different hypertention grades were statistically significant(P<0.05).Among them,LVMI,cIMT and serum leptin,SIRT3 levels of the patients with hypertension stage 3 were significantly higher than that of patients with hypertention stage 1 and 2,while serum SLR level was lower than that of patients with hypertention stage 1 and 2;LVMI,cIMT and serum leptin,SIRT3 levels of the patients with hypertension stage 2 were significantly higher than those of patients with hypertention stage 1,while serum SLR level was lower than that of patients with hypertention stage 1.3.Differences among serum LP,SIRT3 levels of patients in different LVMI and cIMT groups were statistically significant(P<0.05),in which serum LP,SIRT3 levels in non-LVH group were significantly higher than in control group,higher in HLVH group than in non-LVH and control groups(P<0.05).Serum SLR was significantly lower in non-LVH group than that of control group,lower in HLVH group than that of non-LVH and control groups(P<0.05);Serum LP,SIRT3 levels of patients in the plaque formation group were higher than those of patients in cIMT thickness group as well as the normal group,while serum SLR level was lower than that of patients in cIMT thickness group as well as the normal group,serum LP,SIRT3 levels of cIMT thickness group were higher than those of patients in the normal group,while serum SLR level of patients in the former group was lower than that of patients in the latter group.4.The LVMI level was linearly positive correlated with serum LP and SIRT3 levels(r values were 0.671,0.698,P<0.05)and linearly negative correlated with serum SLR level(r value was-0.658,P<0.05);meanwhile,the cIMT level was linearly positive correlated with serum LP and SIRT3 levels(r values were 0.725,0.683,P<0.05)and linearly negative correlated with serum SLR level(r value was-0.720,P<0.05).5.Line to LVMI level as the dependent variable,multiple stepwise regression analysis showed that LP,SLR and SIRT3 entered regression equation.6.Multivariate Logistic regression analysis showed that SIRT3 and LP were independent risk factors for cIMT thickening(OR values were 5.678,1.247,P<0.05),while SLR was an independent protective factor for cIMT thickening(OR value was 0.921,P<0.05).Conclusion: Serum LP,SLR and SIRT3 levels are closely associated with hypertension and cIMT,which indicates the possible participation of leptin resistance and SIRT3 in the development and progression of EH and AS,providing a new serological perspective to evaluate the severity of hypertensive complications and possessing potential value.
