| Objective: To investigate the 2013 edition of BI-RADS-US,the appraisal method on different ultrasonic elastography(grading method of BI-RADS ultrasonic elasticity of 2013 edition,strain ratio method,area ratio method)and the application value in term of the combined method in diagnosis of benign and malignant breast mass.Methods: Based on the conventional ultrasound,98 patients and 103 breast nodules in total which confirmed by pathology were checked by elastic imaging method and classificated with BI-RADS.Build ROC curve compared with pathological result and getting the best diagnostic critical point of strain ratio method and area ratio method.Using different ultrasonic elasticity assessment methods to diagnosis the breast nodules.Adjust the BI-RADS classification according to the different elasticity evaluation results and the discrepancy of different approaches was made by contrast.Results:1.The accuracy of BI-RADS classification diagnosis of breast masses was 94.17%,the sensitivity was 93.85% and the specificity was 94.74%.2.The accuracy of ultrasonic elastography grading method,strain ratio method and area ratio method was 70.87%,82.52%,82.52%,the sensitivity was 90.24%,90.91% and 90.91%,the specificity was 58.06%,72.92% and 72.92%,respectively.3.BI-RADS combined with ultrasonic elastography grading method,strain ratio method and area ratio method,respectively.The accuracy was 91.26%,90.29% and 89.32%,the sensitivity was 93.55%,93.44%,91.94%,specificity was 87.8%,85.71% and 85.37%,respectively.4.The area under the curve of BI-RADS classification,ultrasonic elastography grading method,elastic strain ratio method,area ratio method,BI-RADS combined with ultrasonic elastography grading method,strain ratio and area ratio respectively was 0.952,0.778,0.843,0.892,0.951,0.965,0.952.Conclusion:1.BI-RADS of 2013 edition,the different appraisal method for elastography used solely,the combined method has diagnostic value of breast masses.2.When using different BI-RADS classification method independently,the ultrasonic elasticity grading method has certain limitation on diagnosis.As semi-quantitative evaluation methods,the strain ratio method and area ratio method which can to some extent avoid the subjective impact of physicians thus have higher diagnostic value.3.Two-dimensional ultrasound is the basis of diagnosis,elastic imaging technology can be added as a conventional ultrasound,provide more reference information for the BI-RADS classification and enhance the diagnostic confidence of ultrasound doctors.providing better service for clinical diagnosis and treatment. |
