The Application Research About Combined Automated Breast Ultrasound System(ABUS)and Ultrasonic Elastography On BI-RADS Category3-4 Breast Lumps

Objective:to examine 30 casUE of breast lump with conventional ultrasonography(ABUS)and ultrasound elastography(UE)with conventional ultrasound(US)and BI-RADS 3-4,and compare with the conventional ultrasound examination rUEults,and explore the value of ABUS and UE in improving the diagnostic value of BI-RADS 3-4 breast lumps.The aim is to provide a scientific basis for guiding future clinical application.Methods:From September 2017 to January 2018,the patients with breast lumps in the Second Affiliated ultrasound department of Kunming Medical University were examined first by US,then the US examination was classified between the 3-4 groups,and the subjects were in accordance with the standard.Then the ABUS and UE examinations were taken respectively,and 30 cases with the final pathological results were collected.Based on the pathological results as the gold standard,the routine ultrasound results of 30 cases of breast masses were compared with those of ABUS,UE and ABUS combined with UE.Results:The pathological examination results of 30 breast lesions in 30 patients:benign group were 21 lesions,including 13 fibroadenoma,2 sclerosing adenosis,2 mammary ductal dilatation with atypical ductal hyperplasia,2 intraductal papilloma,1 benign lobed tumor,and 1 cystic hyperplasia with interstitial tissue cell aggregation.In the malignant group,there were 9 lesions,7 invasive carcinoma(invasive non special type of carcinoma,6 invasive ductal carcinoma of breast,1 infiltrating special carcinoma,1 invasive ductal carcinoma),1 ductal carcinoma in situ and 1 real papillary carcinoma.Conventional ultrasound diagnosis showed 22 malignant,8 benign,88.89%sensitivity,33.33%specificity,and a correct index of =0.22.The results of ABUS examination:9 lesions appeared on the coronal plane of ABUS,8 cases were malignant,1 cases were benign and intraductal papilloma,and 21 of the lesions on the ABUS coronal surface did not appear "convergence sign",20 cases proved to be benign,1 cases were malignant and real papillary carcinoma.The sensitivity,specificity and accuracy of ABUS in diagnosing breast tumors were 88.89%,95.24%and =0.84 respectively.The results of four kinds of ultrasound elastic imaging examination:El:5 points and 4 scores were diagnosed as malignant,22 benign,8 malignant,diagnostic sensitivity 77.78%,specificity 95.24%,and correct index =0.73;VTI:6 grade classification,?-grade ? to be benign,?-? grade classified as evil,21 benign,9 malignant and diagnostic spirit.Sensitivity 77.78%,specificity 90.48%,correct index =0.68;VTQ:SWV=3.05m/s as the critical value,more than 3.05m/s as malignant,less than the benign,23 benign,7 malignant,diagnostic sensitivity 11.11%,specificity of 71.43%,the correct index =-0.17;VTIQ:SWV=3.68m/s critical value,larger than 3.68m/s for malignant,less than less than 3.68m/s For benign,22 benign and 8 malignant lesions were found.The sensitivity of the diagnosis was 66.67%,the specificity was 90.48%,and the correct index was=0.57.It can be seen that in the single diagnostic test,the specificity and the correct index of ABUS increased significantly compared to the conventional ultrasound.In the four UE imaging,the sensitivity and the correct index of the VTQ are not as good as that of the conventional ultrasound,and the specificity is higher than that of the conventional ultrasound,but it is still at a lower level compared with the other UE imaging.The sensitivity of EI,VTI and VTIQ was not as good as that of conventional ultrasound,and the specificity and accuracy index were higher than those of conventional ultrasound.When ABUS and UE take joint experiments,the sensitivities of Ba U&E,Ba U&V T,Ba U&V T,ah and V TIQ,compared to single test(ABUS),are all increased,but the specificity is low,the correct index El is improved,and the others are reduced.Conclusion:1.ABUS coronal "convergence sign" has high sensitivity,specificity and correct index,which is of great significance for the identification of benign and malignant type 3-4 breast lumps.2.The diagnostic accuracy of EI,VTI and VTIQ is higher than that of conventional ultrasound,which can provide more information for the diagnosis of breast masses.3.When combined with ABUS and UE,the accuracy of diagnosis can be improved significantly,and the BI-RADS classification of conventional ultrasonography can be optimized in the 3-4 types of lesions,providing more accurate results.Compared with conventional ultrasound,it has statistical significance and has good clinical application prospects.
