| Objective To retrospectively analyze patients with hemifacial spasms who were treated in our hospital from October 2014 to October 2015 to explore the clinical value of EMG in the diagnosis of hemifacial spasm.Methods Forty-three patients with hemifacial spasms who were treated in our hospital from October 2014 to October 2015 were selected as the object of study.The diagnostic criteria of clinical diagnostic criteria were discussed.According to the inclusion criteria to select the case,and finally included 30 patients.Electron electromyography was used to detect facial nerve conduction latency,amplitude,blink reflex and facial nerve innervation in patients with hemifacial spasm.The relationship between facial nerve and blood vessel was observed and compared with that of brain MR imaging.Sensitivity and specificity were observed.EMG examination was divided into two parts: nerve conduction examination and needle electromyography.The former(the electrode placed in front of the ear)refers to the nerve stimulation of the facial nerve,thus recording nerve or muscle electrical activity.The latter refers to the needle placed in the muscles(orbicularis oculi muscle,frontalis,orbicularis oculi muscle)to record its electrical activity.Results There was no significant difference in the latency and amplitude of early response(R1)and late reaction(R2,R2 ’)between the 1,30 patients and the contralateral side.The amplitude of the ipsilateral late reaction was significantly higher than that of the contralateral side,But only to stimulate the same side of the late reaction difference was statistically significant(P <0.05).(71.2 ± 20.3)Hz and(69.4 ± 17.7)Hz,respectively,compared with the control group(P <0.05).(97.6 ± 21.4)Hz and(93.5 ± 17.2)Hz,respectively,which were basically the same(P> 0.05),and the EMG values of the contralateral and ipsilateral were respectively(5.1 ± 3.1)m V and(6.8 ± 4.3)m V,the difference was statistically significant(P <0.05).(103.2 ± 16.4)Hz and(94.2 ± 15.3)Hz,respectively,and the contralateral side of the orbicularis oculi muscle was significantly higher in the 3,30 patients with facial spasm(P <0.05).The MPF values of the contralateral and ipsilateral were(124.3 ± 16.3)Hz and(112.4 ± 14.6)Hz,respectively,and the contralateral was significantly higher than that of the ipsilateral.The difference was statistically significant(47.2 ± 41.2)m V and(36.3 ± 26.4)m V,respectively.The difference between the two groups was significantly higher than that of the affected side(P <0.05).The difference was statistically significant(P <0.05).There were 6 cases of suspicious contact in the affected area and 17.0%(21/30)in 15 patients with definite contact or oppression.The positive rate was 70.0%(21/30)The positive rate was 20.0%(6/30),the positive rate of the affected area was significantly higher than that of the contralateral side,the difference was statistically significant(P <0.05),and the positive rate was 20.0%(6/30)<0.01).5,30 cases of hemifacial spasm in patients with simultaneous brain MR examination and electromyography found that 30 patients,the EMG abnormalities in 23 cases,the positive rate was 76.7%(23/30),brain MR Abnormal 21 cases,the positive rate was 70.0%(21/30),both abnormalities in 15 cases,the positive rate was 50.0%(15/30).Conclusion EMG has important application value in the diagnosis of vascular spasmodic spasms,which can provide a reliable reference for clinical treatment and can be widely used in clinical medicine. |
