| Objective:To research the correlation of adiponectin、copeptin and hepcidin with metabolic syndrome and obstructive sleep apnea hypopnea syndrome,clarify the role of adiponection、copeptin and hepcidin in the pathogenesis of OSAHS.And to explore whether it can provide clinical evaluation,prognosis and efficacy of OSAHS patients.Method:This study randomly selected 40 patients(including mild 30 cases,moderate 5 cases,severe 5cases)from April 2015 to November 2015 at the second hospital of Lanzhou University Diabetes Divison was diagnosed with OSAHS.At the same time,30 cases of healthy people without snoring were selected as the normal control group at the second hospital of Lanzhou University medical center.Then all observed subjects were divided into 4 groups:control subjects,mild OSAHS,moderate OSAHS,severe OSAHS.The levels of serum adiponectin、copeptin and hepcidin were measured by enzyme-linked immuno sorbent assay(ELISA),at the same time,two indicators of sleep were monitored,the sleep apnea hypopnea index(AHI)and pulses oxygen saturation(SPO2).Results:1.TC、LDL、SBP、DBP、UA、BMI、FBG、Fins、HOMA-IR、copeptin and hepdicin in patients with OSAHS were higher than in normal control group(p<0.05),HDL and adiponectin levels in OSAHS group were lower than those in the normal control group(p<0.05),but there is no significant difference in gender、age and TC between OSAHS patients and normal control group.2.AHI is positive correlated with Hb A1c、BMI、FBG 、Fins and HOMA-IR in OSAHS patients(r=0.49,P=0.01;r=0.33,P=0.02;r=0.45,P=0.01;r=0.38,P=0.02;r=0.62,P=0.01),and it is negative with LSPO2(r=-0.35,P=0.02),but there are no related with TG、HDL、LDL 、SBP、DBP and UA(r=0.14,P=0.34;r=0.13,P=0.39;r=-0.09,P=0.55;r=0.11,P=0.56;r=-0.31,P=0.07;r=0.28,P=0.07).3.The levels of adiponectin is decreased in patients with severe OSAHS compared with mild and moderate OSAHS patients,but the level of copeptin and hepdicin are significantly higher.4.The serum adiponectin is positively correlated with LSPO2(r=0.41,P<0.01),but negatively correlated with AHI and HOMA-IR(r=-0.87,P<0.01;r=-0.46,P<0.01),but there is no correlation with Fins(r=-0.27,P>0.05).And the levels of copeptin and hepdicin in OSAHS patients are positively correlated with AHI(r=0.75,P<0.01;r=0.0.88,P<0.01),but negatively with LSPO2(r=-0.63,P<0.01;r=-0.30,P<0.05).5.Draw the receiver-operating characteristic curves(ROC)of serum adiponectin copeptin and hepdicin levels,the serum adiponectin concentration is 8.02μg/ml as the threshold for the diagnosis of OSAHS,the area under the ROC curve is 0.9±0.04,95% confidence interval is 0.83~ 0.97,sensitivity is 73%,specificity is 100%,and copetin concentration is 8.63pmol/l as the threshold for the diagnosis of OSAHS,the area under the ROC curve is 0.97±0.02,95%confidence interval is 0.93~1.00,sensitivity is 97%,specificity is100%,and hepdicin concentration is 482.32pg/ml as the threshold for the diagnosis of OSAHS,the area under the ROC curve is0.97±0.02,95% confidence interval is 0.93~1.00,sensitivity is 97%,specificity is100%.Conclusions:1.OSAHS patients are often associated with hypertension、hyperglycemia、hyperrlipidemia、high uricacid、obesity、insulin resistance and other factors.2.Serum adiponectin,copeptin and hepdicin can determine the severity of OSAHS patients and adiponectin is a protective factor for OSAHS.3.Serum adiponectin、copeptin and hepdicin are likely to be useful in predicting biological markers for OSAHS patients. |
