Correlation Between The Serum Copeptin And Clinical Severity In Patients With Obstructive Sleep Apnea Hypopnea Syndrome

Objective:To explore the changes of Copeptin levels and the correlation between the serum copeptin(CPT)and clinical severity and significance in patients with Obstructive Sleep Apnea Hypopnea Syndrome(OSAHS).Methods:1.A total of 87 subjects were selected in the Respiratory Sleep Monitoring Center of Binzhou Medical College,by Polysomnograph(PSG)detection and diagnosis for OSAHS patients and health people,recording the apnea hypopnea index(AHI),mean oxygen saturation(M-Sp02),Lowest oxygen saturation(L-Sp02)and other parameters.According to their AHI scores,OSAHS patients were divided into three groups:mild OSAHS(5 ≤ AHI<15,n=17),moderate OSAHS(15≤AHI<30,n=19)and severe OSAHS(AHI≥30,n=36).2.A11 subjects were recorded the age,gender,height,weight,BMI,neck circumference,waist circumference,hip circumference,past medical history.3.All subjects were collected 3-5ml of cubital venous blood after overnight fasting,copeptin and CPR levels were measured with enzyme-linked immunosorbent assay.And Total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),High density lipoprotein cholesterol(HDL-C),lipoprotein a(LPa),Creatinine(Cr),Uric Acid(UA)biochemical indicators were measured in the laboratory of our hospital.Results:1.A significant association was found between patients with OSAHS and copetin and CRP levels,neck circumference,waist circumference,ESR,ALT,TC,TG,LVEDD、LVEF(P<0.05);There was no difference in age,gender,hip circumference,hypertension(HBP),diabetes(DM),smoking history,drinking history,WBC,AST,UA,HDL-C,and LDL-C in the patients with OSAHS compared to simple snoring(P>0.05).2.We found that copeptin and CRP levels、BMI and AHI in the three groups of patients OSAHS were significantly higher than that of the simple snoring,but the L-SpO2 was lower(P<0.05).Comparison between groups,copeptin levels were significantly higher in mild OSAHS than in controls,moderate OSAHS were higher than mild OSAHS(P<0.05);But serum copeptin levels were no statistically significant difference between moderate OSAHS and severe OSAHS.3.Copetin levels showed a significant positive correlation with Neck circumference(r=0.337,P<0.01),BMI(r=0.406,P<0.01)、AHI(r=0.578,P<0.01)、CRP(r=0.382.p<0.01),TG(r=0.329,P<0.01),LVEDD(r=0.346,P<0.01).But it was negatively correlated with L-SpO2(r=-0.369,P<0.01),LVEF(r=-0.221,P<0.05).It had no correlation with age、sex and M-SpO2.4.Multivariate linear regression analysis showed that there was a linear regression relationship between the level of Copeptin and AHI,and the standardized regression coefficient was 0.59、0.22(P<0.05).Conclusion:We found that copeptin levels were significantly higher in OSAHS patients,and correlated positively with the severity of OSAHS,which was closely related to the occurrence of OSAHS,and the disorder of lipid metabolism and inflammation in patients with OSAHS.It can affect ventricular remodeling and cardiac function in OSAHS patients,which may be a clinical index to judge the severity of OSAHS and provide reference for clinical risk stratification.
