Objectives Taking Hb A1 c as a standard of dividing groups.According to compare the clinical data about the rate of peri-implantitis and the change of hard and soft tissue arounding the dental implants.The purpose of this study is to provide the the oretical basis and data support for the implant maintenance and the range of blood glucose control.Methods The slection of testing people: According to include and exclude standards.All the patients were selected from January 2014 to July 2016 in our hospital.Among of them,48 patients are type-2 DM patients and 36 patients are healthy.the patients were informed before the research project and before the test they would sign the informed consent All the subjects according to the average of every three month's(3 ? 6 ? 9 ? 12)Hb A1 c were divided into three groups: group A(Hb A1 c ? 6.0)with 36 cases,group B(6 < Hb A1 c ?8.0)with 30 cases,and group C(8.0<Hb A1c<10.0)with 18 cases.Experimental methods: Recording the age and gender about the patients.Testing the concentration of blood glucose before the surgery and controlling it under the6.7~12.2mmol/L.After consulting the treatment plan,a periapical film was taken.Implant placement surgery was taken under minimally invasive protocol: incision was made and flap was elevated.The implant bed was prepared according to the surgical manual of the implant company.The implant was then placed and wound sutured.The implant was then placed and wound sutured.After 4 months' healing period,the final prosthesis was delivered.During follow ups,the probing depth(PD),marginal bone level(MBL)and modified sulcus Bleeding Index(ms BI)were measured and incidence of peri-implantitis was recorded.Statistical method:The statistical method of PD and MBL using one-way ANOVA,and the comparing of percentage using Chi-square,the comparing of score of ms BI using rank-sum test.All the clinical data were analysed by statistical software like SPSS19.0.Results 1 There is no significant difference between three groups about the age and gender(P>0.05).2 There is no significant difference in the changes of PD between A,B,C six months and one year after restoration in different Hb A1 c range.(P>0.05).3 There is no significant difference in MBL between the three groups of A,B and C Six months and one year after restoration,the MBL of group C was higher than that of group A and group B(after six months P_A=0.041,P_B=0.035;after one year P_A=0.021,P_B=0.032),the difference was statistically significant(P<0.05).4 There is no significant difference in ms BI between the three groups of A,B and C Six months and one year after restoration,the ms BI of group C was higher than that of group A and group B(after six months P_A=0.016,P_B=0.028;after one year P_A=0.034,P_B=0.031),the difference was statistically significant(P<0.05).5 There is significant difference between the three groups about the incidence of periimplantitis(P=0.048).6 There is no significant difference between three groups about the rate of success(P>0.05).Conclusions 1 The rate of success osseointegration is higher,but high level of HbA1 c will affect its bone' losing.2 With the increasing of level of Hb A1 c,it will arouse the possible of blooding by probing.3 The short-term(about one year)result of implant surgery in DM patients under the good control can be successful,while long-term result need more observation.4 The rate of peri-implantitis increased with the increase of Hb A1c's level,its effects on soft and hard tissue around dental implants increased.
