Objective:Spontaneous bacterial peritonitis(SBP)is a common life-threatening complication in patients of hepatic cirrhosis with ascites,and the in-hospital mortality rate of the disease can reach to 10%-50% in spectet of the continuous improvement of its therapeutic regimen.The current study aims to explore the association among the changes of single nucleotide polymorphism(SNP)of genes of nucleotide-binding oligomerization domain-containing protein 2(NOD2)(R702W,G908 R,P268S),Monocyte chemotactic protein-1(MCP-1)(A2518G)and the susceptibility,profiles ofvarious lab examination of patients with hepatitis B cirrhosis associated SBP,in order to search new biomarkers for the early diagnosis and treatment,prognostic prediction for patients with SBP related to hepatitis B cirrhosis.Methods: A total of 80 hospitalized patients of hepatitis B cirrhosis with ascites from January 2015 to June 2016 at Dalian Sixth People 's Hospital,were enrolled in the study,among them 40 cases with spontaneous bacterial peritonitis(SBP group)and others without SBP(non SBP group).In addition,30 healthy individuals were selected as normal controls(control group)..Samples of peripheral blood from each one of all groups and the ascites from patients were collected and processed for total blood genomic DNA extraction,target gene amplification(PCR),DNA fragemt digestion and identification by digestion with restriction endonucleases and agarose gel electrophoresis.The changes of three NOD2 SNP(R702W,G908 R,P268S)and one SNP of MCP-1 promoter(A-2518G)were analyzed and the gene types were compared with patients clinical data and the report of lab examinations including Serum Albumin,Total bilirubin,liver function classification,Serum Sodium and Serum Procalcitonin.Statistical analysis of the SNP of NOD2 and MCP-1 genes,and lab examinations,classification of liver function was carried out bwtween groups of normal and hepatitis B cirrhosis,patients complicated with or without SBP.Results: When cpmparing three amplification products(R702W,G908 R,P268S)of NOD2 we did not find the variant types of G908 R and P268 S among all three groups,and only two cases of pateints with SBP(2/40,0.05%)showed the variant types of R702 W which was not been observed in normal control and patients without SBP.Regarding the varant SPN types of MCP-1 promoter(A-2518G),we observed three genotypes distributed distinctively in different groups: SBP group with significant higher ratio of genotype AG than that of both non-SBP group and healthy control group(?2=10.95,P=0.004);the ratio of GG genotype in SBP group was also much higher than that of non-SBP patients(?2=24.78,P<0.001).By comparing the different alleles,we observed that the frequences of allele AG in SBP group were significantly higher than that in non SBP group,P<0.05,Non-SBP group of genotype GG was higher than group SBP,P<0.05;allele comparison:allele A in SBP group was significantly higher than that in non-SBP group,P<0.05;allele G in SBP group was significantly higher than that in group SBP,P<0.05;alleles of G were higher than healthy control group of 18 patients in the SBP group and the distribution frequency non SBP group(30%),P<0.05,the difference was statistically significant.By linking the clinical data of with the SNP varants,we observed that there were statistically significant differences(P<0.05)in the Non-SBP group and SBP group in terms of Serum ALB,TBIL,liver function,Serum Sodium,PCT statistically significant differences of P<0.05,however there were no significant difference in the two groups regarding to the other clinical features such as gender,age,Prothrombin time,Alanine Aminotransferase,Aaspartate aminotransferase.Conclusion:1.There were no difference of NOD2 SNP(G908R,P268S)between normal individual and hepatic cirrhosis patients with or without althogh 2 SBP patients showed NOD2 genes SNP R702 W that is not statistically significant,suggesting that NOD2 three gene SNP(G908R,P268 S,R702W)were not the biomarker determining the susceptibility of SBP in80 Han population who suffered from hepatitis B cirrhosis with ascites.The ratio of MCP-1(A2518G)AG genotype and A allele increased in Han population of SBP associated with hepatitis B cirrhosis with ascites.Carrying AG genotype and A allele in patients with hepatic cirrhosis with ascites after a comprehensive clinical evaluation should be treated as soon as possible.2.the levels of Serum ALB,TBIL,Serum Sodium,PCT and Child-Pugh grade C were the risk factors of the occurrence of Hepatic Cirrhosis complicated with spontaneous bacterial peritonitis.
