Study Of Distribution Characteristic Of TCM Constitution And Echocardiography In 106 Patients With Obstructive Sleep Apnea Syndrome

Objective:To investigate the distribution and characteristics of epidemiology and TCM Constitution in patients with Obstructive Sleep Apnea Hypopnea Syndrome(OSAHS)who lived in Lanzhou district of Gansu province.The parameter of cardiac form and hemodynamics of OSAHS patients were detected by echocardiography,and evaluate the changes and extent of the damage of cardiac structure and function in patients with OSAHS.Meanwhile,the characteristics and rules of the change not only in OSAHS patients but also in all TCM Constitution were compared,the purpose is to provide the indicatio n for judging and curative effect of OSAHS disease in the future direction.Methods:106 patients suffered from OSAHS were examined by medical history,physical examination and polysomnography monitor,40 cases was treated as normal control group which the age,sex were matched.The basic data,TCM Constitution and ESS sleepiness questionnaire were surveyed,and the sleep monitoring were carried at the same time.Then,to analyze the epidemiological characteristics and the distribution of TCM Constitution in patients with OSAHS by the clinical epidemiological investigation method.Finally,the related parameters were detected by echocardiography and discuss the features of cardiac structure and function in patients with OSAHS.Results:(1).OSAHS patients with smoking history,drinking history,high body mass index,hypertension and dyslipidemia were higher than those in the control group(P<0.05).(2).The distribution characteristics of OSAHS which including the light,moderate,severe are as follows: the prevalence rate of male was higher than female,and the high incidence of age was 35-60 years,The combination of smoking history and higher body mass index increased the proportion of patients,who were more serious(P<0.05).(3).106 cases of OSAHS patients,mainly TCM Constitution types are Phlegm-dampness Constitution,Q i-deficient Constitution,which accounted for 33.02%,24.53%.followed by Yang deficiency,Balanced Constitution,Q i stagnation Constitution,Damp-heat Constitution,Yin deficient Constitution,I nherited special Constitution and Stagnat Blood Constitution which accounted for 10.48%,9.43%,7.55%,6.60%,4.72%,1.88% and 1.88%(P<0.05).(4).The patients with Phlegm dampness Constitution and Q i deficiency Constitution are more severe and the patients with Balanced Constitution,Yin deficiency Constitution,Damp heat Constitution and Q i stagnation Constitution were relatively mild.The male is mainly composed of Balanced Constitution and Phlegm dampness Constitution,and the female is Yang deficiency Constitution and Q i stagnation Constitution.The young people are mainly based on the damp-heat Constitution,while the middle aged people are mainly composed of Q i deficiency Constitution and Phlegm dampness Constitution.The patients with phlegm dampness,damp heat and Qi stagnation often have a history of smoking,patients with phlegm dampness have more obesity,and Balanced Constitution,Q i deficiency and Yang deficiency have more overweight.The proportion of Q i deficiency,Phlegm dampness,Damp heat Constitution and Q i stagnation in urban population are higher than that in rural(P<0.05).(5).With the aggravation of the OSAHS,the levels of LVEF,FS,E/A were lower,and LVEDD right ventricular diameter,diameter of main pulmonary artery,right ventricular wall thickness and pulmonary artery systolic pressure increased(P<0.05).It also showed that LVEDD,right atrial diameter,right ventricular diameter,main pulmonary artery diameter,right ventricular wall thickness and systolic pulmonary artery pressure in Phlegm dampness Constitution and Qi deficiency Constitution are higher than Balanced Constitution(P<0.05).Conclusions:(1).OSAHS patients often complicated with a variety of cardiovascular risk factors,and exacerbated with the severity of illness.(2).The main type of constitution of OSAHS patients in Lanzhou region are Phlegm dampness Constitution and Q i deficiency Constitution;Different TCM Constitution and different degrees of OSAHS patients have the corresponding distribution characteristics,so it can be predicted the OSAHS prevalence and progress through the TCM constitution identification,in order to guide the clinical diagnosis and treatment.(3).OSAHS can cause left and right heart structure and function changes,which are more obvious with the aggravation of OSAHS condition.The structure and function abnormalities can be detected dynamically by echocardiography,which can reflect the extent of cardiac damage of OSAHS.
