| Objective: Suppurative infection wound,was also known as nonspecific infection,which is more difficult to be healed,as accompanied by local tissue blood supply.Western medicine always used wet compress of antibiotics,which can not solve the problem of local blood supply;long-term use of antibiotics would easily produce drug resistance;and could lead to prolonged course.Diabetic ulcer was one of the serious complications of diabetes.Conventional dressing effect was very little,easily leading to secondary infection and gangrene,as well as persistent disease,could be up to several months to decades,the prevalence of the elderly persons was particularly high,could cause hospital length,the cost was very large,the death and amputation rate was very high,which was one of the chronic refractory ulcers.Traditional Chinese medicine occupies a unique position because of its significantly clinical efficacy,lower disability.With the development of modern society,a variety of traumatic diseases was increasing,open trauma,wound infection have become the most common complications of traumatic diseases.At present,Western medicine promoting wound healing was in poor efficacy,and resulting in more complications.Traditional Chinese medicine have relatively better effect and lower adverse reactions.Therefore,to evaluate the efficacy,safety and economy of compound fluid of Cortex Phellodendri in the treatment of suppurative infection of soft,diabetic ulcer,traumatic infection,safety and economical efficiency,we conducted clinical trials research,which is of great significance.Methods: The clinical trial was selected by clinical trial center,then the patients were divided into experimental group and control group,and randomized,single-blind,positive control clinical trial were adopted.Taken the Jingwanhong ointment and Kangfuxin liquid as controls,design the dosage programs,then evaluate the Traditional Chinese medicine syndrome curative effect,ulcer ulcer(or wound)healing rate and ulcer area and its single symptom curative effect(such as sore area,depth,granulation,wheezing,etc.),the wound healing time,the wound granulation indicators etc.and safety analysis.And by the economic evaluation and related data processing software processing and statistical testing,we obtained the results.Results: Compared with the control group,the effective rate of Compound Fluid of Cortex Phellodendri was 98.61% in the treatment of soft tissue suppurative infection,which was higer than Jingwanhong ointment 58.33%(P <0.05),the ulcer healing rate was 86.11% better than Jingwanhong ointment 61.11%(P < 0.05).The ulcer area of Compound Fluid of Cortex Phellodendri before treatment(2.65 ± 2.34)cm2 and after treatment(1.45 ± 1.07)cm2 was significantly higher than that of the control group before and after treatment was(2.51 ± 2.42)cm2 and(1.56 ± 1.13)cm2(P<0.05)respectively,indicating that the Compound Fluid of Cortex Phellodendri group in the ulcer area treatment effect is better than than Jingwanhong ointment;In the treatment of diabetic ulcer comprehensive efficacy,the total effective rate was 100.00% in the treatment group better than the Kangfuxin liquid 72.22%(P <0.05).The ulcer healing rate was 89.81% in the Compound Fluid of Cortex Phellodendri group.The control group was 77.78%(P <0.05).From the 7th day after treatment,improvement of the wound area of the Compound Fluid of Cortex Phellodendri was better than the Kangfuxin liquid.The improvement effect was improved to the 28 th day;The total effective rate was 96.11% higher than that of Kangfuxin liquid group(68.33%)(P <0.05).On the 7th day after treatment,the wound healing rate of Compound Fluid of Cortex Phellodendri was higher than that of Kangfuxin liquid(P <0.05).The main symptoms of soft tissue suppurative infection,diabetic ulcer and traumatic infection(sore area,perirenal swelling,sore depth,Pills,granulation growth situation);secondary disease(sore heartburn,thirst)and other aspects of the score found that compound Fluid of Cortex Phellodendri than the Jingwanhong ointment and the Kangfuxin liquid control group effect is significant.This result indicated that compound Fluid of Cortex Phellodendri could notably improved the clinical symptoms of suppurative infection of soft tissue,diabetic ulcer and traumatic infection.Through the cost-effect analysis,the fluid of Cortex Phellodendri has more economic advantage compared with other control drugs.Conclusion: In the clinical treatment of suppurative infection of soft tissue,compound fluid of Cortex Phellodendri can effectively improve the syndrome of soft tissue,wound healing rate,wound area,wound healing time,wound signs and subjective symptom;In the clinical treatment of diabetic ulcer,compound fluid of Cortex Phellodendri promote the wound healing process and relieve wound infection and the degree of pain with patient;In the clinical treatment of traumatic infection,compound fluid of Cortex Phellodendri was effective in wound healing time,healing rate and the granulation growth,and safe in clinical use and safe in clinical use.Therefore,in the clinical treatment of suppurative infection of soft tissue,diabetic ulcer and traumatic infection,the compound fluid of Cortex Phellodendri has good clinical application prospect. |
