Objective: This study measured shear wave velocity(SWV)of pancreas in healthy people and patients with type 2 diabetes mellitus(T2DM)by virtual touch tissue quantification(VTQ),in order to realize the digital quantification of pancreatic elasticity,through statistical analysis,the aim is to explore the clinical value of VTQ on detecting pancreas in healthy people and patients with T2 DM and provide a new technical method for the clinical to assess the pathological changes of pancreatic parenchyma accurately and objectively.Methods:96 patients with T2 DM diagnosed by our hospital endocrinology from October 2015 to October 2016 were selected as the experimental group,and 116 healthy persons were selected as the control group in the same period of physical examination.Acuson S3000 color ultrasonic diagnostic instrument was used to observe the shape,size,boundary and internal echo of the pancreas,and measure the anterioposterior diameters of head,body and tail of pancreas.Start VTQ imaging technique and places the sampling frame in the region of interest(ROI)of the pancreas to obtain the SWV and Depth.At the same time,All subjects were detailed height,weight,total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL),low density lipoprotein(LDL),fasting blood glucose(FPG),systolic blood pressure(SBP),diastolic blood pressure(DBP)and duration of diabetes.Results:1 Comparison of clinical data between the control with T2 DM group:BMI,TC,TG,SBP,DBP of T2 DM group were higher than the control group but HDL was lower than the control group.The difference was statisticallysignificant;There were no statistical significance between the two group in age,gender and LDL.2 Comparison of two dimensional ultrasound images of pancreas between T2 DM group with control group: The images of pancreas in control group appeared: normal size,homogeneous echo,smooth margin.The images of pancreas in T2 DM group appeared: the atrophy of volume,heterogeneous echo,unsmooth margin.The anterio-posterior diameters of pancreatic head,body and tail of T2 DM group were smaller than those of the control group,and the difference was statistically significant.3 Measurement of SWV values of pancreas in control group3.1 The correlation analysis: The SWV values of the head and body of pancreas were not correlated with gender,BMI,TC,TG,HDL,LDL,SBP,DBP,the anterio-posterior diameters of pancreas,The SWV values of the head and body of pancreas were positively and significantly associated with age.3.2 Comparison of SWV values between different groups classified by age in the control group:The SWV values of pancreas of each group classified by age was statistically significant different.3.3 Comparison between different positions of pancreas in control group:There is no significant difference of SWV values among head,body of pancreas in control group.4 Comparison between control group with T2 DM group classified by age:The SWV values of pancreas in T2 DM group was significantly higher than that of the control group corresponding to different age.5 Measurement of SWV values of pancreas in T2 DM group5.1 The correlation analysis: The SWV values of the head and body of pancreas were not correlated with gender,BMI,TC,TG,HDL,LDL,SBP,DBP,the anterio-posterior diameters of pancreas,The SWV values of the head and body of pancreas were positively and significantly associated with duration of diabetes.Only the SWV values of the head of pancreas were positively and significantly associated with age.5.2 Comparison between different group classified by the duration of disease in T2 DM group: The SWV values of pancreas of each group classified by the duration of disease was statistically significant different.Conclusion:1 There are differences between the control and T2 DM group in shape,size,margin,internal echo of the two dimensional ultrasound images of pancreas.There has some clinical value in the detection and diagnosis of T2 DM through conventional ultrasonography.2 The SWV values of pancreas of healthy people were measured by VTQ technique,which provided the normal reference values for further study of SWV values of other diseases of pancreas.3 VTQ can be used to assess the pathological changes of pancreatic parenchyma accurately and objectively in patients with T2 DM and may be a new criterion of digital quantification.
