Direct Method CT Venous Imaging, Color Doppler Ultrasonography Joint D-dimer Testing Of Lower Extremity Deep Vein Thrombosis Diagnosed Clinical Significance

Objective: To study the direct method idose reconstruction of low doses of multislice CT angiography(MSCTV),color doppler ultrasound(CDS)imaging examination method combined with detecting peripheral blood D-dimmer concentration in diagnosis of lower extremity deep vein thrombosis(DVT).Methods: We collected 128 cases of DVT patients from February 2014 to October2014 in imaging department clarified explicit pathogenesis of DVT of each patient and relevant medical history affecting the measurement result of D-dimmer.We recorded results of MSCTV,CDS and peripheral blood of D-dimmer test of each patient.Then we compared the similarities and differences of peripheral blood D –dimmer concentration in different parts of the body and different duration of thrombosis.In addition,we compared the advantages and disadvantages of two kinds of imaging technology.Results: There were differences for concentration of D-dimmer in different classification of DVT.The concentration of peripheral blood D-dimmer in central and mixed type was 4.45±1.2 mg/L?2.48±0.24 mg/L significantly higher than the normal range.However,the concentration of peripheral blood D-dimmer in peripheral type was around 0.41 ± 0.12mg/L,close to the normal range.Direct method MSCTV,CDS,imaging techniques and the sensitivity of the determination of plasma D-dimmer,specificity of 99.2%,98.4%,100%,83.3%,80%,60%,positive rate and negative rate and accuracy of 99.2%,99.2%,98.4%,88.3%,66.7%,100%,98.4%,97.6%,98.4%;MSCTV and CDS in the diagnosis of the sensitivity of the around the type,central type,and mixed type respectively96.9%?93.8%,92.6%?66.7%,93.8%?84.4%.Conclusion: In view of the fact that plasma D-dimmer has extremely high sensitivity,it can be used as the exemplary indexes of DVT.Direct method MSCTA and CDS diagnosis of the DVT have the extremely high consistence.Whereas,colordoppler ultrasound in discovery of pelvic vein and vein thrombosis between crus muscle was less than vein angiography using CT.On the other hand,CT vein angiography couldn't observe hemodynamic change and venous valve function damage.In conclusion,these three detection approach should be applied jointly to reduce misdiagnosis and missed diagnosis of DVT.
