The Application Effect Of Padua Scale And Modified Padua Scale In VTE Risk Assessment Of Trauma Surgery Patients

Objective: Evaluation of Padua scale and modified Padua scale for risk assessment of venous thromboembolism in trauma patients by retrospective review of patient records.Methods: The data of 590 hospitalized patients with traumatic surgery admitted to the Affiliated Hospital of Qingdao University from November 2015 to September 2016 were retrospectively analyzed.Among them,25 patients were diagnosed with VTE.Caprini scale,Padua scale,modified Padua scale and RAP score were used to evaluate 590 patients respectively.Through analysis the results of score: evaluation of the assessment for the diagnosis in patients with high risk for venous thromboembolism in the proportion of patients,compare the rates of construction;designthe receiver operating characteristic curve(ROC)model,compare the sensitivity,specificity and the Youden index of modified Padua scale,Caprini scale,Padua scale and RAP score.Results: A retrospective study of 590 hospitalized patients with trauma surgery.Evaluation of Caprini scale for the positive rate of high risk patients accounted for 6.93%,Padua scale evaluation for the positive rate of high risk patients accounted for 8.75%,the modified Padua scale positive rate accounted for 8.42%,compare each other showed no significant difference(P>0.05);by constructing ROC curve model : area Caprini scale,Padua scale,modified the Padua scale and RAP score evaluation of trauma surgery in patients with venous thromboembolism risk under the curve(AUC)were 0.808(95%CI=0.754~0.862),0.751(95%CI=0.676~0.826),0.773(0.700~0.847)and 0.774(95%CI=0.687~0.0.860),the optimal cut point(Caprini scale is more than 3.5 points,Padua scale is more than 3.5 points,the improved Padua scale is more than 4.5 points,RAP score is more than 6.5 points).The sensitivity was 100%,96%,96% and 80%,the specificity was 55.4%,55.8%,56.1% and71.5% respectively,the Youden index were0.554,0.518,0.521 and 0.515;Caprini and Padua scale and modified Padua scale,RAP score both have are ignificant difference in the efficacy of venous thromboembolism in patients with traumatic surgery(P<0.05).Conclusion: The Padua scale and the modified Padua scale can be used as a tool to evaluate the risk of venous thromboembolism in trauma surgery patients.
