Objectives A case-control study was conducted to study the relationship between Caprini RAM evaluation and the risk of VTE in COPD patients receiving preventive anticoagulant therapy.To evaluate the application value of Caprini RAM in COPD patients receiving preventive anticoagulant therapy.To explore the risk factors of VTE in COPD patients receiving preventive anticoagulant therapy.In order to provide some theoretical support and evidence-based medicine for the application of Caprini RAM in COPD patients.Methods The survey was conducted mainly through case-control study.Chronic obstructive pulmonary disease patients who received prophylactic anticoagulant therapy of low molecular weight heparin(LMWH)were investigated.They were all hospitalized in the medical ward of North China Polytechnic University Hospital from December 2013 to December 2017.51 patients were found to be diagnosed with venous thromboembolism.Afterwards,the 51 patients were taken as the case group.Then we selected 153 patients without venous thromboembolism for the same period as the control group.Used the general situation questionnaire to record the medical record information of the two groups of patients.And a uniform Caprini RAM was used to score the included cases and classify the venous thromboembolism risk levels of them according to the medical records.After comparing the basic data of the two groups of patients by chi-square test,we compared the Caprini RAM score and the risk levels between the two groups.The relationship between Caprini RAM evaluation and the risk of VTE was analyzed.And these VTE influencing factors were analyzed and compared with univariate analysis.Multivariate analysis using logistic regression analysis.As for data analysis,? = 0.05 was used to determine whether the test was statistically significant.Results 1 There was no statistically significant difference in the basic data such as age,gender and BMI between the two groups(P>0.05).2 The Caprini RAM score in the case group was significantly greater than in the controls(P<0.05).3 The Caprini RAM score ? 6.0((?) =6.07)was used as the exposure factor.Otherwise,it was non-exposed.The relationship between the Caprini RAM score and the risk of VTE was analyzed.The result showed that the risk of VTE in the exposed population was 5.463 times that of the unexposed population,which was statistically significant(P<0.05).4 The risk levels of the two groups were found to be in the medium-risk and above.The proportion of patients classified as very-high-risk level in the case group was higher than that in the control group,and the difference was statistically significant(P<0.05).The risk of VTE in very-high-risk level patients was 2.808 times that of medium and high-risk patients(P<0.05).5 Univariate analysis was performed on influencing factors of VTE,and the results showed that bed rest,swollen legs(current),congestive heart failure(within 1 month),pulmonary infection(within 1 month),history of prior major surgery(within 1 month)and VTE history were related to the occurrence of VTE(P<0.05).6 The six factors that were statistically different in univariate analysis together with Caprini RAM score were incorporated in the multivariate analysis.Binary Logistic regression analysis showed that in COPD patients receiving preventive anticoagulant therapy,Caprini RAM score ? 6.0,bed rest,swollen legs(current),history of prior major surgery(within 1 month),VTE history were closely related to the occurrence of VTE,among which,Caprini RAM score ? 6.0 is an independent risk factor for VT,and the higher the Caprini RAM score,the greater the risk of VTE in patients.Conclusions In COPD patients receiving preventive anticoagulant therapy,there is a close relationship between Caprini risk assessment scale evaluation and the risk of venous thromboembolism.The higher the Caprini RAM score and risk level,the greater the risk of VTE in patients.Caprini RAM has high application value in such patients.
