| Objective: Observe the effect of the teeth,which were treated by apicoectomy without retrograde filling,on the healing of large jaw cysts in anterior,and provide the basis for choosing treatment plans in the future.Methods: 25 patients with jaw cysts from 2013.1~2015.3 treated in oral and maxillofacial surgery of affiliated hospital of Qingdao university.14 were males,11 were females,the mean age was 42 years old and the mean diameter was 28 mm.Divide them into two groups,one was periapical cysts and another was other types of jaw cysts,which in light of history,clinical examination and image examination.To compare the differences of the two groups,we measured the largest diameter and the numbers of bone defect(BD)in terms of the preoperative CT.Clinical evaluation and radiographic evaluation were made after 6 months,1 year and 2years,then access the final clinical effects in terms of the clinical evaluation and radiographic evaluation.Through periapical X-ray and CBCT,compare the index of relative bone healing,which was the quotient of relative densities in BD and the surrounding area.The diameter and the index of relative bone healing were statistically analyzed using independent-samples t test.The numbers of BD and the clinical effect were analyzed by Wilcoxon rank-sum test.Statistical value was set on p>0.05.Results: Among the 25 patients,12 belonged to the group of periapical cysts and 13 belonged to other types.There was no difference of the diameter and the numbers of BD.After two-year follow-up,3were complete healing,20 were incomplete healing,1 was uncertain healing and 1was failure.Overall,23 were success,and the success rate was 92.1%.1was effective and the effective rate was 4%.1 was failure and the failure rate was 4%.In the group of periapical cysts,2 was complete healing,9 was incomplete healing and1 was failure.Then 11 were success and the success rate was 91.7%,1 was failure and the failure rate was 8.3%.In the group of other types,1 was complete healing,11 were incomplete healing and 1was uncertain healing.Finally12 were success and the success rate was 92.3%,1 was effective and the effective rate was 7.7%.There was no difference of the clinical effects(P=0.91>0.05).Besides,there was no difference of the index of relative bone healing.Conclusion:(1)In the premise that the involved teeth were treated by adequate root canal therapy and coronal restoration,large periapical cysts and other types of jaw cysts in anterior can get good clinical effects,in which the involved teeth were treated by apicoectomy without retrograde filling.(2)Because of severe bone destruction,the followup should be longer than 2 years for large jaw cysts. |
