Objective To investigate the spatial distribution characteristics of asymptomatic dotlike hemosiderin spots and lacunes(non-liability lesions)in the brain of patients with cerebral infarction,and correlate the severity of dotlike hemosiderin spots,lacunes(non-liability lesions)and leukoaraiosis.Method The study retrospectively assessed 109 consecutive inpatients diagnosed with cerebral infarction and underwent brain MRI examinations,including SWI sequence,the clinical data,blood biochemical examination result,the number of dotlike hemosiderin spots on SWI sequence,the number of lacunes(non-liability lesions)on T1 sequence or T2 sequence were collected.The total number was divided according to the severity of dotlike hemosiderin spots and lacunes(non-liability lesions),their clinical data and blood biochemical examination result were compared.To investigate the distributions,the number of dotlike hemosiderin spots and lacunes(non-liability lesions)in different part of the brain were compared,and correlate their consistency.The distribution of dotlike hemosiderin spots in different parts of the brain was examined statistically,as well as the lacunes.Correlation analyses were done on the relation between the different grades of dotlike hemosiderin spots,lacunes(non-liability lesions),and leukoaraiosis.Result There was no significant difference between the three groups with varying degrees of dotlike hemosiderin spots and lacunes(non-liability lesions)at clinical data and blood biochemical examination result(P>0.05).Dotlike hemosiderin spots and lacunes(non-liability lesions)were found at various foci in the brain,with a preference for the cortico-sudcortico,then basalganglia,thalamus,brain stem,cerebellum.There were significant differences between the numbers of dotlike hemosiderin spots and lacunes(non-liability lesions)in cortico-sudcortico,basalganglia,thalamus,brain stem(P value were <0.001,<0.001,0.002,0.003 respectively,P<0.05 with statistical difference),except in cerebellum(P>0.05).There was a weaker positive correlation between the distribution of the dotlike hemosiderin spots and lacunes(non-liability lesions)in basalganglia,thalamus(Kappa value were 0.297,0.371 respectively,P value were <0.001,<0.001 respectively).The distribution of dotlike hemosiderin spots in different parts of the brain is inequality,so is the lacunes.There was a significant positive correlation between the grades of dotlike hemosiderin spots and the grades of the lacunes(non-liability lesions)(Spearman's correlation coefficient: r = 0.351,P<0.001),the grades of leukoaraiosis were positively correlated with those of the dotlike hemosiderin spots(r = 0.475,P<0.001),and also with those of the lacunes(r = 0.360,P<0.001).Conclusion This study found that the dotlike hemosiderin spots was most common in the cortico-sndcortical region and basal ganglia,but the cortico-sndcortical region was the most common site for the lacunes in patients with cerebral infarction.There were significant differences in distribution of dotlike hemosiderin spots and lacunes(non-liability lesions)in cortico-sudcortico,basalganglia,thalamus,brain stem,except in cerebellum,and the distribution of both in basalganglia,thalamus are consistent,but are weakly consistent.This study also found that the severity of the dotlike hemosiderin spots was positively correlated with the severity of lacunes(non-liability lesions),and both types of lesion were closely correlated with the degree of leukoaraiosis.This study speculate embolus extravasation is an alternative mechanism for the formation of dotlike hemosiderin spots,and when the endothelial function is damaged,the emboli can't be translocated,which resulting in persistent tissue hypoxia,synaptic damage and cell death,the dotlike hemosiderin spots and the lacunes(non-liability lesions)are different result of small vessels dealing with the emboli.
