| Background: Leukocyte count is an important means of diagnosing joint disease,and even in some diseases(such as culturing negative joint infections)as the only diagnostic indicator.The existing white blood cell counting methods are manual counting method,instrumental method and leukocyte lipase semi-quantitative count.Many studies suggest that the white blood cell count manual method and instrumental method in the counting results are not significantly different,so automatic blood analyzer has been widely used in clinical laboratories.But our clinical experience has found that there are errors in the instrument method count.Therefore,we discuss the consistency of the two counting methods for leukocyte counting results.Leukocyte lipase as a white blood cell count of semi-quantitative indicators of enzymes,has been widely used in urine,ascites,cerebrospinal fluid and other body fluids,Parvizi et al in 2011 for the first time in the joint infection in the diagnosis of infection,the current diagnostic accuracy Sex is still controversial,so the relationship between white blood cell lipase and white blood cell count also need further study.Objective: To investigate the correspondence between the results of XE-5000 automatic blood analyzer and manual counting in the application of white blood cell count in synovial fluid.The relationship between the results of leukocyte esterase test paper and the results of white blood cell count of synovial fluid was studied to improve the accuracy of clinical diagnosis of joint disease And convenience.Methods: Collected from November 2016 to February 2017 in our hospital due to joint disease need to artificial knee,hip replacement of patients with synovial fluid samples of 50,of which 4 due to less amount of synovial fluid,not included in the study,a total of 46 synovial fluid specimen.The XE-5000 blood automatic detection analyzer and manual method were used to calculate the number of white blood cells in the synovial fluid.The difference between the two groups was linear regression analysis and the t-test of the matched samples,and the consistency was compared.In addition to take 0.5ml specimens,drop in the white cell ester enzyme test strips,2-3 minutes after the observation results.If the bloody specimens,the titration before the centrifuge centrifuge 5000 r / min centrifugal treatment for 2-3 minutes,take the supernatant titration test paper,read the results.To observe the difference between the results of the interpretation and the results of white blood cell count.To determine the correspondence between the results of leukocyte esterase test paper and the results of white blood cell count of synovial fluid.In this study,the manual counting results were compared with the gold standard.Results: 1.The white blood cell count was 13685.70 ± 33526.69 / ?L and the manual white blood cell count was 10120.04 ± 27196.05 / ?L.There was no significant difference in the white blood cell count between instrument and manual method(P> 0.05)).The linearity of the two counting methods is good(R = 0.821),and the manual counting result is slightly lower than the instrument method.2.There were 12 counts of two counts in the joint samples,and the results were inconsistent.Among the patients,4 cases were femoral neck fractures(80% of the patients)and 4 cases were femoral head necrosis(P <0.05).There were significant differences between the two groups(P <0.05).There were significant differences between the two groups(P <0.05).Suggesting that in these diseases,the two methods often appear a high rate of inconsistency,automatic blood analyzer accuracy is poor.3.Leukocyte lipase and white blood cell count corresponding to the results: "-" corresponds to 0-266 / ?L,"±" corresponds to 266-616 / ?L,"+" corresponds to 616-1330 / ?L,"++" corresponds to 1330-2478 / ?L,greater than "++" corresponds to a white blood cell count greater than 2478 / ?L.Leukocyte lipase and white blood cell count results have a good correspondence.Leukocyte lipase in the process of reading,of which four specimens due to mixed with bloody,still can not be interpreted after centrifugation.Conclusion: 1.The results of the XE-5000 automatic blood analyzer and the manual counting method on the white blood cell count of the joint fluid are not significantly different in the overall situation,have good consistency,and the manual counting result is slightly lower than the instrument method.2.Instrument method and manual method in the femoral head necrosis,femoral neck fractures and revision surgery in patients with poor consistency in the results of white blood cell count,the need for manual counting method review.3.Leukocyte lipase test paper test and manual joint fluid white blood cell count has a good correspondence between the diagnosis of clinical joint disease in a better reference. |
