Objective The H.pylori infection rate of Chinese general people has been as high as 50%-80%.Andit is increasing at the rate of 1%-2% every year.Meanwhile the injection involves in the occurrence and development of many other diseases.With further research,people find that H.pylori has a close relationship with insulin resistance and glycolipid metabolism.The current study on the relationship between H.pylori infection and insulin resistance is mostly on some clinical diseases,such as diabetes,cardiovascular diseases and so on.In my study,I made a detailed research about the affection that H.pylori infection has on insulin resistance and other metabolic indicators.The study Is aimed at people who are healthy and people who do not have pre-disease symptoms after physical examination.And the investigation of the affection that H.pylori infection has on blood glucose,blood lipid and insulin resistance was also done.Methods In my study,620 cases were selected randomly among the people who got physical examination in Health Examination Center of Tianjin Medical University Healthcare Department from January 2016 to October 2016.Some cases were removed because of incomplete data.The cases were also abandoned such as hypertensive patients who were receiving treatment,patients who had high blood lipid,diabetes or hyperuricemia,patients who had cardiovascular diseases and severe liver-kidney dysfunction.The cases,that had been diagnosedas H.pylori infectionor infected with H.pylori inrecent one year,could not be included either.Some other cases were removed,such as patients that had received acid suppression treatment or were receiving treatment of proton pump inhibitors or H2 receptor blockers in recent three months,patients who received antibiotic treatment,patients who had malignant neoplasms,patients who would have surgery recently,patients who were in pregnancy and lactation and patients who had mental illness or acute gastrointestinal bleeding.Finally 594 cases were chosen in my study.The age of these cases is from 20-83.And the average age is 52.2±12.7 These cases consist of 273 male ones and 321 female ones.Blood samples were taken to detect blood glucose,blood lipids and biochemical indicators and then the infection situation were determined through 13C-UBT test.HOMA-IR model was taken to estimate the situation of insulin resistance.Here HOMA-IR=FPG?mmol/L?×FINS?m IU/L?/22.5.The objects of this study were divided into H.pylori positive group and negative group according to the H.pylori infection situation.I analyzed the H.pylori infection situation and contrasted the difference between the two groups about blood sugar,blood lipids and insulin resistance situation.Logistic regression model was adopted to analyze the divided into two groups named abnormal glucose metabolism group and healthy control group according to the result of blood glucose test.And then analysis of insulin resistance situation was done when glucose metabolism was abnormal.The analysis about whether there is difference for insulin resistance,blood glucose and blood lipid level among the abnormal glucose metabolism group and the control group was also done.Results 1.There are 363 cases detected to be H.pylori infected person and the infection rate is 61.1%.The male number of infected ones is 159 and the female number is 204.H.pylori infection does not have statistical significance for gender and age?p>0.05?.If age is divided into different levels,H.pylori infection rate is almost about 60% of each age level and it would not change obviously with age increasing.2.There is no statistical significancefor the differences of blood pressure,AST,ALT,CR,UR between H.pylori positive and negative group?p>0.05?.FBG and Hb A1 c reflecting blood glucose situation of H.pylori positive group are higher than that of H.pylori negative group.And there is statistical significance?p=0.037,p=0.035?.But HDL of H.pylori positive group is much higher and there is statistical significance?p=0.004?.What's more,HOMA-IR reflecting insulin resistance situation has obvious difference between two groups.So they both have statistical significance through chi-square test.3.For Logisticregression analysis,HOMA-IR is as the dependent variable.Multiple stepwise regression analysis was done with H.pylori,age,BMI,TG,LDL-C,HDL–C,2h PG and Hb Alc as independent variableafter the factor FBGand FINS were removed.From the result we can see that H.pylori infection,BMI and Hb A1 c are risk factors of HOMA-IR and the OR values are respectively 8.664,1.251 and 1.784?P<0.05?.Meanwhile HDL is the protective factors of HOMA-IR and the OR value is 0.225?P<0.05?.Conclusion Inthrough this study,a survey about H.pylori infection situation of healthy physical examination population was done.By this survey,I explored the relationship between H.pylori infection situation and the changes of blood sugar and lipids and relationship between H.pylori infection situation and insulin resistance.From this paper,we find there is a close relationship between H.pytori infection and insulin resistance for the abnormal glucose metabolism cases.By regression analysis,we can see that H.pytori infection is the independent risk factor and H.pytori infection also has an impact on glycolipid metabolism.
