| Objective :IgA nephropathy is one of the most common glomerular diseases in the world which kidney immunopathology is characterized by IgA or IgA-based immunocomplex deposition in the glomerular mesangial area.IgA nephropathy was first described by French scholar Berger and Hinglais in 1968,also known as Berger's disease.The clinical manifestations of IgA nephropathy are diverse.Most patients presented with recurrent episodes of gross hematuria,asymptomatic hematuria and proteinuria,some patients can be combined with edema,hypertension,renal insufficiency.High incidence of IgA nephropathy is a progressive disease,which is the primary cause of end-stage renal disease in China.The pathogenesis of IgA nephropathy is not yet fully elucidated,there is no uniform treatment of clinical programs and special effects drugs,clinical often use symptomatic treatment,depending on the type of pathology and progression of disease selective use of glucocorticoids and/or immunosuppressive agents,but the disease is easy to relapse,and hormones and immunosuppressive agents have a greater toxic side effects.In recent years a large number of experimental studies and clinical practice shows that traditional Chinese medicine in the syndrome differentiation of IgA nephropathy has achieved remarkable results.Professor Tan combined with years of clinical experience,on the basis of syndrome differentiation,combined with individual patients,With the method of invigorating Qi and nourishing yin and clearing heat and removing dampness,using the No.2 prescription of nephritis to treat the disease,has achieved significant clinical efficacy,and provide the theoretical basis and research ideas for TCM treatment of IgAnephropathy.Methods:All cases were selected from the patients of outpatient in Hebei Provincial Hospital of Traditional Chinese Medicine.All cases were selected according to the diagnostic criteria,inclusion criteria and exclusion criteria.All patients were randomly divided into treatment group(n=30)and control group(n=30).All patients were given the same basic treatment(including general treatment and symptomatic treatment).On the basis of foundation treatment,the treated group was treated by the No.2 prescription of nephritis,the control group was given Huang Kui capsule.The course of treatment is 3 months.Observe the changes of urine erythrocyte count,24-hour urine protein quantification,plasma albumin(ALB),Blood lipids(CHOL,TG),renal function(BUN,Scr,UA),blood and urine ?2-microglobulin and clinical symptom score to evaluate the clinical efficacy of the drug.Results:1 The total effective rate of the treatment group was 86.67% and63.33% in the control group.There was significant difference between the two groups(P<0.05).2 The symptom scores of the treatment group had significant difference comparing before treatment(P<0.01).After treatment in the treatment group compared with the control group,there was significant difference(P<0.01).3 From the proportion of pathological grading,Lee's III was the highest,Lee's?was the lowest,there was no significant difference in the therapeutic effect of the three.4 The treatment group was better than the control group in reducing proteinuria,hematuria,elevating plasma albumin,lowering blood lipids,reducing uric acid and lowering blood and urine ?2-microglobulin which was statistically significant(P<0.05).Conclusions:1 With the method of invigorating Qi and nourishing yin andclearing heat and removing dampness,using the No.2 prescription of nephritis to treat IgA nephropathy,can significantly reduce the clinical symptom score and improve the clinical symptoms of patients.The effect is better than the control group.2 The No.2 prescription of nephritis can reduce proteinuria and hematuria,elevate plasma albumin,regulate blood lipids,reduce uric acid,and protect renal function.3 The No.2 prescription of nephritis can reduce serum and urine ?2microglobulin levels,reverse the early renal damage,improve renal function,delay the progress of IgA nephropathy. |
