Study On The Diagnostic Value Of Multi Slice Spiral CT Coronary Angiography In Coronary Heart Disease

Objective: To analyze the accuracy of Multi Slice Spiral CT Coronary Artery Imaging in diagnosis of coronary heart disease and in judgment of artery stenosis degree.Methods: A total of 112 patients that met the inclusion criteria were included in the study,of which 63 were male,49 were female,the average age of 37-78(62 ±15),29 cases of diabetic patients,42 cases of patients with hypertension,53 cases of patients with dyslipidemia,37 cases of patients with hyperuricemia.Of all the subjects,1290 branches of coronary artery were evaluated.CTA examination showed that there were about 354 vessels in the lesion,and CAG examination showed that there were no more than 348 vessels.In the coronary artery CTA examination showed that 354 lesions in the blood vessels,CAG support the presence of lesions in which there are 327 vessels,there are 27 CTA examination to determine the presence of lesions and CAG showed no lesions.For CAG examination showed 348 lesions of blood vessels,of which there are 21 CTA showed no lesions.In the 354 vessels of CTA showed that in the mild stenosis with a total of 85,of which 7 left main coronary artery,left anterior descending branch and a total of 24 branches,31 left circumflex and right coronary artery and its branches were 23;moderate stenosis with a total of 148,of which 15 left main coronary artery,left before to its branches with a total of 46,48 left circumflex and right coronary artery and its branches with a total of 39;severe stenosis with a total of 121,of which 11 left main coronary artery,left anterior to its branches with a total of 32,30 left circumflex and right coronary artery and its branches with a total of 48.In the 348 vessels of CAG showed that in the mild stenosis with a total of 72,of which 5 left main coronary artery,left anterior descending branch and a total of 26 branches,30 left circumflex and right coronary artery and its branches were 11;moderate stenosis with a total of 138,of which 15 left main coronary artery,left the anterior descending branch and a total of 41 branches,42 left circumflex and right coronary artery and its branches with a total of 40;severe stenosis with a total of 138,of which 9 left main coronary artery,left anterior descending branch and a total of 35 branches,40 left circumflex and right coronary artery and its branches with a total of 54.Collected 112 cases of patients in our hospital,they were accepted the inspection of coronary artery CTA,and were accepted the inspection of coronary angiography(CAG)after 2 weeks,referring to the result of the percutaneous selective coronary angiography,the accuracy of CTA in different degree of coronary artery stenosis was evaluated.The degree of coronary artery stenosis: mild stenosis: classification of vascular diameter stenosis less than 50%;moderate stenosis: stenosis of 51-75%;severe stenosis:stenosis more than 76%.The data were collected and analyzed to explore the clinical value of coronary CTA examination in the diagnosis of coronary heart disease.Result: The results were statistically analyzed using SPSS 17 software.Referring to the result of CAG inspection,the sensitivity,specificity,accuracy,positive predictive,negative predictive value of CTA inspection are: mild stenosis 77.78%?97.62%?96.51%?65.89%?98.67%,moderate stenosis 85.51%?97.40%?96.12%?79.73%?98.25%,severe stenosis 68.84%?97.74%?94.65%?78.51%?96.32%,and two methods have no statistical discrepancies in diagnosis of coronary heart disease and in judgment of artery stenosis degree(P>0.05).Conclusion: The diagnostic accuracy of spiral CT coronary angiography is similar to that of percutaneous selective coronary angiography,and it is a noninvasive and reliable method for the screening of coronary heart disease.
