Investigation And Analysis Of Adverse Transfusion Events And Its Influencing Factors

Objective: By summarizing reported adverse events related to blood transfusion in recent four years in our hospital.To analyze the clinical features and influencing factors of adverse events,to inquiry the measures to reduce adverse events related to blood transfusion,and promote scientific and safe blood transfusion.Methods: The data of adverse events related to blood transfusion in our hospital from 2013 to 2016 were collected,Analyze its characteristics and the incidences and distribution of adverse transfusion reactions were further analyzed,the type and proportion of near miss events.Results: A total of 46669 blood transfusion cases occurred in 2013-2016.there were 511 cases of adverse events related to blood transfusion.of which 209 cases the serious hazards of transfusion,both were adverse transfusion reaction,among them,86 cases of anaphylactic reaction accounted for 41.45%,63 cases of non-hemolytic fever transfusion reaction accounted for 30.14%,16 cases of citric acid poisoning accounted for 7.66%,five cases of blood transfusion-related cycle overload accounted for 2.39%,38 unidentified cases accounted for 18.18%.The incidences of adverse transfusion reactions such as suspended erythrocytes,plasma,platelet,and cryoprecipitate were 0.43%,0.40%,0.78%,and 0.53% respectively.The incidences of adverse transfusion reactions in patients that had a history of blood transfusion,allergies,or pregnancy were 61.72%,80.38%,and 84.62% respectively,P <0.05,rash,urticaria,chills,fever were more common;including 302 cases of near miss events,divided into specimen errors,storage and handling errors,clinical errors,53 cases of extracting specimens in wrong collection process,wrong specimen information had 27 cases,unqualified specimens had 24 cases,2 cases of withdrawal specimens;83 cases were unsealed bag during recovering blood bag,the blood box failed 16 cases,53 cases of no blood preparation before surgery,irrational use of blood 26 cases,9 cases were insufficient blood supply,9 cases were not reported blood transfusion adverse reactions;no error accident.Conclusion: In our hospital,the serious hazards of transfusion both were adverse transfusion reaction,Among them,mainly consisted of anaphylactic reaction and non-hemolytic fever transfusion reaction,the blood transfusion-related circulation overload and citric acid poisoning were less.The incidences of adverse transfusion reactions were higher in patients with history of blood transfusion,allergy or pregnancy;For near miss events,unsealed bag during recovering blood bag and no blood preparation before surgery were also frequent,filling wrong blood type were not found.Through analyzing the data of adverse events related to blood transfusion during four years,it was helpful to understand its characteristics,which could improve the ability of medical staff to diagnose and prevent the adverse events related to blood transfusion,it also provide data support to achieve scientific,safe,reasonable and effective use of blood.
