Objective:To determine prognostic factors,predict survival time and seek the optimal treatment modality,elderly patients with locally advanced squamous cell lung cancer have been retrospectively analyzed.Methods:According to eligibility criterion,162 cases of elderly patients with squamous cell lung cancer were selected,from January 1,2010 to January 1,2015 in Shanxi Tumor Hospital.Related prognostic factors were carried on univariate and multivariate analysis with Kaplan–Meier approach and Cox regression,respectively;treatment-related adverse reactions were compared.Results:The median age of 162 patients was 73.6 years old.The overall median survival time was 19.4 months,the 1-year survival rate was 71%,and the-2 year survival rate was35.9%.In univariate analysis with Kaplan-meier analysis method: age(?2=7.94,P=0.005),ECOG score(?2=42.12,P=0.000),chemotherapy combined with radiotherapy or not(?2=14.99,P=0.000)were prognostic factors that influenced survival.COX regression analysis showed that ECOG score(HR=0.299,P=0.000,95%CI: 0.193 ~0.462)and N stage(HR=0.645,P=0.026,95%CI: 0.439 ~ 0.949)were independent prognostic factors.Treatment-related adverse reactions: the cases of adverse reactions of chemoradiotherapy group were higher than those of radiotherapy group in hematological system,digestive system and respiratory system;toxic reactions especially in leukocyte(P=0.000),thrombocyte(P=0.001),alimentary canal(P=0.000)and acute lung inflammation(P=0.005)had statistical significant(P< 0.05)by chi-square test between the two groups.Conclusion:1.ECOG score and N stage were independent prognostic factors of the elderly patients with inoperable locally advanced squamous cell lung cancer.The lower the ECOG score(0-1 score)is,the less region lymph node metastasis(N0-1)has and the longer it survives,so is the opposite.2.The elderly patients with locally advanced squamous cell lung cancer were more likely to have side effects when treated with chemoradiotherapy.
