The Research Of Open Reduction And Plate Fixation For Treatment Of Calcaneal Fractures

Objective:The purpose of this study is to research the efficiency of the treatment of calcaneal fractures using open reduction and plate fixation.Methods:Using retrospective analysis,from January 2010 to April 2016,44 patients with calcaneal fracture were treated with open reduction and plate fixation in the first hospital of Fujian medical university,including 35 men,9 females,aged16-58 years,mean age 41.82�10.02 years old,.all of patients were taken CT scan and three-dimensional reconstruction before the operation.According to Sanders classification system,there are 8 patients with 8 calcaneal fracture belong to the Sanders type II,11 patients with 12 calcaneal fracture belong to the Sanders type III,25 patients with 26 calcaneal fracture belong to the Sanders type IV,all of them were taken surgical treatment of open reduction plate fixation.In pre-operation and post-operation,respectiely measured the calcaneals' length,width,heigth,the anlge of B?hler and Gissane in the lateral axis X-ray in the types of Sanders II?III?IV,and evaluated the condition of the reduction of fratures.In post-operation,outpatient review and telephone follow-up for 12 month to 85 months,evaluating the function of the foot by Maryland foot score system,and the data were analyzed by SPSS22.0 statistical software.When P <0.05,it was statistically significant.Results:44 patients with 46 calcaneal fractures achieve the follow-up actually.Follow-up time 12 months to 85 months,the mean follow-up was about(38.00 � 25.00)months.The last follow-up fracture were clinically healed,no nonunion appeared.2cases occur wound infection,the wounds were healed after dressing treatment and sensitive antibiotic use.3 cases occur subtalar joint inflammation,we suggest the patients take joint fusion arthrodesis.The other patients' foot function recover well.In pre-operation and post-operation,respectiely measured Sanders II?III?IV calcaneals' length,width and heigth,the anlge of B?hler and Gissane in lateral calcaneal slice,and the results used T-test,there was significant difference(P<0.05).According to the standard of Maryland score system,for Sanders type II,7 feet got an excellent result,1good,0 satisfactory,0 poor,the rate of fine was 100%.In the type of Sanders III,9 feet got an excellent result,2 good,1 satisfactory,0 poor,the rate of fine was 91.7%.The type of Sanders IV:10 feet got an excellent result,8 good,5 satisfactory,3 poor,the rate of fine was 80.8%.Conclusion:The surgical treatment in calcaneal fractures using open reduction and internal fixation can get satisfictic anatomical reduction and has achieved good results.
