To Explore The Expression Of HE4,CA125 And CA199 In Ovarian Mature Teratoma,Ovarian Endometriosis And Other Benign Tumors Of The Ovary

Objective:To analyze the expression of serum HE4,CA125 and CA199 in ovarian mature teratoma,ovarian endometriosis and other benign ovarian tumors,and compared with HE4,CA125 and CA199 alone and combined detection for mature teratoma,ovarian endometriosis and other benign ovarian tumor diagnostic sensitivity and specificity for ovarian,serum tumor specificity and more sensitive markers for mature ovarian teratoma,ovarian endometriosis and other benign ovarian tumor diagnosis,in order to improve the diagnostic level.And the analysis of mature teratoma,ovarian endometriosis patients serum CA125,CA199 levels and the correlation between the expression of HE4 in ovary,changes of serum HE4,CA125 and CA199 before and after treatment were compared,the clear index for mature teratoma,ovarian endometriosis and other benign ovarian tumor diagnosis and treatment and prognosis evaluation of clinical significance of ovary.Methods:From January 2015 to January 2016 Ganzhou People's Hospital and Ganzhou Municipal Hospital for female patients with pelvic masses need surgery as the research object.All patients with preoperative and postoperative collected serum HE4,CA125 and CA199 numerical,and selected according to the results of the postoperative pathological diagnosis of 90 cases included in the statistical analysis.There were 30 cases of ovarian mature teratoma,30 cases of ovarian endometriosis,and 30 cases of benign ovarian cancer.The pathological diagnosis as the standard for four table analysis,sensitivity and specificity were calculated;the receiver operating characteristic(receiver operating,characteristic,ROC)curve,calculate the area under the curve(area under the curve,AUC),to reflect the accuracy of diagnosis.Mann-Whitney U test was used to test the difference between three population mean values and Pearson correlation analysis was conducted.The changes of serum HE4,CA125 and CA199 before and after operation were compared and analyzed.Result:1.group of mature ovarian teratoma and ovarian endometriosis group serum HE4 were significantly higher than those of other ovarian benign tumor group patients,the difference was statistically significant(t=3.917,4.094,P < 0.05),were no significant difference between the groups of mature ovarian teratoma and ovarian endometriosis group(t= 0.015,P > 0.05);while the CA125 level between groups comparison,only the ovarian endometriosis group and other ovarian benign tumor group there was significant difference among ovarian endometriosis group was higher than that of other ovarian benign tumor group,t=2.203,P < 0.05;while the CA199 level of comparison between the three groups showed no significant difference(P >0.05).2.according to the postoperative pathological diagnosis,other benign ovarian tumor group is the reference population.The sensitivity(97.68%)of HE4+CA125+CA199 combined detection in ovarian teratoma was the best,but the specificity(88.69%)was decreased.The sensitivity and specificity of HE4 were the highest(90.49% and 91.35%)respectively.3.by correlation analysis,the level of CA125 and HE4 expression has a strong correlation,r=0.907,P < 0.01,suggesting that with the elevation of CA125 level,HE4 was expressed at high level;also there is a certain correlation between the expression level of CA199 and HE4,r=0.769,P < 0.01,suggesting that with the elevation of CA199 level,HE4 expression was also increased.4.the three groups of patients after the serum HE4,CA125,CA199 levels have declined,and compared with before surgery,the difference between the groups statistically significant,P < 0.05.Conclusion:1.serum HE4 diagnosis and differential diagnosis of ovarian mature teratoma,ovarian endometriosis value is higher,can be used as a screening index.2.serum HE4 is positively related to the expression level of CA125,but there is a positive correlation between CA199 levels and HE4 expression.The expression level of serum HE4 is more accurate than that of CA125 and CA199 in the diagnosis of ovarian mature teratoma and ovarian endometriosis3.Serum CA125,CA199 and HE4 levels can be used as a reference for clinical diagnosis and prognosis evaluation of ovarian benign tumors.
