Objective To investigate the pathogeny of Hashimoto's thyroiditis(HT)and the relationship between renal function and HT.In order to provide clinical basis for the prevention of misdiagnosis.simultaneously,we also study the inflammatory marker in the serum of HT patients.Which provided the basis for exploring the pathogenesis of HT.Methods(1)A questionnaire survey was conducted on 400 outpatient and inpatient with Hashimoto's thyroiditis in the Second Affiliated Hospital of Suzhou University to explore the risk factors associated with the pathogenesis of HT.(2)We detected the cytokines and costimulatory molecules in peripheral blood of HT patients to explore their roles in the pathogenesis of HT.(3)The serological examination of the thyroid function and the renal function of 1270 patients with Hashimoto's thyroiditis were analyzed retrospectively.Results were analyzed by Graphpad Prism 5 software and Spearman correlation analysis was used.The patients were divided into the normal and the higer level group according to the level of TSH.Two-sample t test was used to analysis the differences of C3,C4,Cr,urea,uric acid,IgA,IgG and IgM between two groups.A probability value of P < 0.05 was considered as statistically significant.Results(1)The risk factors of Hashimoto's thyroiditis could be iodine intake,stress and negative emotion,among which pressure accounts for a very high proportion.(2)The level of IL-2?IL-6?IFN-? in peripheral blood in HT patients was significantly higher than that in control group IL-2(3.814.07 vs.1.92±0.68),IL-6(3.34±2.06 vs.2.12±1.59),IFN-?(2.85±3.98 Vs.1.53±0.82).The level of IL-10 in peripheral blood in HT patients was significantly lower than that in control group(1.61±0.53 Vs.2.25±1.18)There was no significant difference between the two groups in the level of IL-4?TNF-? and IL-17A(P >0.05);(3)The level of CEA was significantly higher than those of normal controls and the level of sB7-H3 was significantly lower than those of normal controls.(4)A change of level of C3,C4,Cr,uric acid,urine latent blood,IgG and IgM in higer TSH level group statistically significant was observed in comparison with the the normal group(P <0.05).There is a correlation between TSH levels and renal function in the higer level group.Cr is positively correlated with TSH.IgG?IgM is positively correlated with TPOAb?TGAb and TSH.C3 is negatively correlated with TPOAb?TSH.C4 is negatively correlated with TPOAb.Conclusion1 HT is more common in 20-40 year old female,the level of antithyroid autoantibody were reduced in patients who took selenium yeast.2 The Th1/Th2 imbalance exist in HT patients and the secretion of cytokines was usually Th1 based.3 Renal function is correlated with thyroid function in patients with thyroid Hashimoto.Thyroid disease may lead to abnormal renal function.
