Clinical Analysis Of Descemet’s Membrane Detachment Repairment After Anterior Chamber Paracentesis Combined With Gas Injection(One Case Report)

Objective:To review the case and related literatures,summarize the factors,clinical features and treatment of Descemet’s membrane detachment after phacoemulsification,analyze diagnosis standard,new treatment way and prevention strategy.To understand the related factors of Descemet’s membrane detachment,which can provide a reference for early diagnosis,intervention and rational treatment.Methods:The medical records of the patients undergoing repairment of Descemet’s membrane detachment in ophthalmology center of the 2nd Affiliated Hospital of Zhejiang University from September 2015 to August 2016 were collected,we followed-up Descemet’s membrane by using anterior segment optical coherence tomography,and evaluated the effect of the operation.Through the literature retrieval of the database,we made a thorough analysis of the related research progress of Descemet’s membrane detachment after phacoemulsification.Results:From September 2015 to August 2016,there was one case of Descemet’s membrane detachment after cataract surgery,with one eye,the left eye underwent phacoemulsification and intraocular lens implantation in the local hospital.On the first day after operation,the Descemet’s membrane detachment was observed.And anterior chamber injection of air was used to repair.The day postoperatively,the Descemet’s membrane detachment remained,the repairment was failed.An alternative technique-anterior chamber paracentesis combined with gas injection for repairment was used,and it worked,corneal transparence was achieved.Conclusion:1.Descemet’s membrane detachment can be a serious complication that may lead to corneal decompensation after phacoemulsification.2.Small detachments can self repairment,mild detachment can use gas or viscoelastic repairment,larger detachments need to take corneal suture,and the anterior chamber paracentesis combined with gas injection repairment for progressive detachments after routine treatment,this alternative technique provides a novel and effective method of repairment for complicated and severe Descemet’s membrane detachment.3.Early diagnosis and active treatment of Descemet’s ’membrane detachment are necessary to restore corneal transparency and improve postoperative visual acuity.
